疝囊高位结扎术与传统开放手术治疗儿童疝的临床研究

Clinical study of high ligation of hernia sac in children and traditional open surgery in the treatment of hernia in children

目的对比分析疝囊高位结扎术与传统开放手术治疗儿童腹股沟疝的临床效果。方法选取2017年1月至2020年7月在保定市妇幼保健院和北京儿童医院保定医院确诊治疗的腹股沟疝患儿共68例,根据入院时间前后(分界点为2020年11月左右)将其均分为研究组和常规组,每组患儿34例。常规组患儿实施传统开放手术进行治疗,研究组患儿实施小儿疝囊高位结扎术进行治疗,对比分析2组患儿的手术情况及临床治疗效果等。结果研究组手术时间、术中出血量、恢复时间、住院时间均明显少于常规组,差异有统计学意义(P<0.05);研究组的总有效率(100.00%)显著高于常规组(94.12%),差异有统计学意义(P<0.05);研究组的复发率2.94%(1/34)显著低于常规组14.70%(5/34),差异有统计学意义(P<0.05);研究组的并发症总发生率1例(2.94%)显著低于常规组6例(17.65%),差异有统计学意义(P<0.05)。结论疝囊高位结扎术较传统开放手术治疗儿童腹股沟疝更加安全可靠,术中出血量小,恢复速度快,并发症少。

Objective To compare and analyze the clinical effect of high ligation of hernia sac and traditional open surgery in the treatment of hernia in children.Methods From January 2017 to July 2020,68 children with hernia who were identified and treated at Baoding Maternal and Child Health Hospital and Baoding Hospital,Beijing Children's Hospital were recruited and separated into study groups according to the period of arrival(the cut-off point was around November 2020),and the regular group,each including 34 children.The children in the regular group underwent conventional open surgery,while those in the study group underwent high ligation of the hernia sac.Comparing and analyzing the surgical circumstances and clinical treatment outcomes of the two groups.Results The operation time,intraoperative blood loss,recovery time,and hospitalization time of the research group were significantly shorter than those of the conventional group,and the difference was statistically significant(P<0.05);the total effective rate(100.00%)of the research group was significantly higher than that of the conventional group(94.12%),the difference was statistically significant(P<0.05);the recurrence rate of the study group 2.94%(1/34)was significantly lower than that of the routine group 14.70%(5/34),the difference was statistically significant(P<0.05).The total incidence of complications in the study group[1 case(2.94%)]was significantly lower than that in the routine group[6 cases(17.65%)],and the difference was statistically significant(P<0.05).Conclusion Compared with traditional open surgery,high ligation of the hernia sac in children is safer and more reliable,with less intraoperative blood loss,faster recovery,and fewer complications.
