闭合复位经皮空心钉内固定对髌骨骨折患者膝关节功能及并发症的影响

Effect of Closed Reduction and Percutaneous Hollow Nail Internal Fixation on Knee Joint Function and Complications in Patients With Patella Fracture

目的探讨闭合复位经皮空心钉内固定对髌骨骨折患者膝关节功能及并发症的影响。方法将髌骨骨折88例患者纳入研究,根据奇偶生成法分为对照组(44例)与观察组(44例)。对照组患者接受切开复位克氏针张力带固定手术,观察组患者接受闭合复位经皮空心钉固定手术。对比两组患者术前与术后6个月的疼痛程度、膝关节功能,并记录并发症发生情况。结果手术前,两组患者的VAS评分与Lysholm评分对比差异无统计学意义(P>0.05);手术后两组患者VAS评分均下降,Lysholm评分均升高,且观察组优于对照组,差异有统计学意义(P<0.05);观察组并发症发生率6.82%与对照组22.72%相比,差异有无统计学意义(P>0.05)。结论针对髌骨骨折患者使用闭合复位经皮空心钉固定治疗,与外科微创原则相符合,能够固定骨折部,对软组织的损害较小,可有效缓解患者的疼痛感,促进其膝关节功能恢复,安全性较高,具有推广意义。

Objective To investigate the effect of closed reduction and percutaneous hollow nail internal fixation on knee joint function and complications in patients with patella fractures.Methods 88 patients with patella fractures were included in the study.According to the parity generation method,they were divided into a control group(44 cases)and an observation group(44 cases).The control group received open reduction Kirschner wire tension band fixation,and the observation group received closed reduction percutaneous hollow nail fixation.The pain degree and knee joint function of the two groups before and 6 months after the operation were compared,and the complications were recorded.Results Before surgery,there was no significant difference between the two groups of patients’VAS scores and Lysholm scores(P>0.05);after surgery,the VAS scores of the two groups decreased,while Lysholm scores increased,and the observation group was better than the control group.There was statistical significance(P<0.05);the complication rate of the observation group was 6.82%(3/44)compared with 22.72%(10/44)in the control group,and there was no statistical significance(P>0.05).Conclusion The use of closed reduction and percutaneous hollow nail fixation for patients with patella fractures is consistent with the principle of minimally invasive surgery.It can fix the fracture and cause less damage to the soft tissues.It can effectively relieve the pain of the patient and promote the recovery of knee joint function.The safety is high,and it has promotion significance.
