芬太尼分别复合地佐辛、酮咯酸氨丁三醇静脉自控镇痛对LC术后患者疼痛及镇静程度的影响观察

The Effect of Fentanyl Combined With Dezocine and Ketorolac Tromethamine on the Pain and Sedation of Patients After LC

目的观察芬太尼复合地佐辛与芬太尼复合酮咯酸氨丁三醇静脉自控(PCIA)镇痛对腹腔镜下胆囊切除术(LC)术后患者疼痛及镇静程度的影响。方法采用随机数字表法将我院收治的98例LC患者分为研究组与对照组,每组各49例,其中对照组患者使用芬太尼复合地佐辛镇痛,研究组患者使用芬太尼复合酮咯酸氨丁三醇镇痛。比较两组患者术后1h、2h、4h、8h、12h、24h、48h的疼痛程度[视觉模拟评分法(VAS)]及镇静程度(Ramsay镇静评分),记录两组患者镇痛期间不良反应发生情况。结果术后1h、2h、4h、8h、12h、24h、48h,研究组患者VAS评分低于对照组,但差异无统计学意义(P>0.05)。术后1h、2h、4h、8h、12h、24h、48h,两组患者Ramsay镇静评分比较差异无统计学意义(P>0.05)。两组患者均未出现呼吸抑制等不良反应,研究组患者不良反应总发生率为16.32%,显著低于对照组的38.78%(P<0.05)。结论芬太尼复合酮咯酸氨丁三醇静脉自控镇痛与芬太尼复合地佐辛静脉自控效果相似,均具有较好的镇痛、镇静效果,但芬太尼复合酮咯酸氨丁三醇不良反应更少。

Objective To observe the effect of fentanyl combined with dezocine and fentanyl combined with ketorolac tromethamine(PCIA)on the pain and sedation of patients after laparoscopic cholecystectomy(LC).Methods A total of 98 LC patients in the hospital were divided into study group and control group by random number table method,each with 49 cases.Patients in the control group used fentanyl combined with dezocine for analgesia,and patients in the study group used fentanyl combined with ketorolac tromethamine for analgesia.Compare the pain degree(VAS)and the degree of sedation(Ramsay sedation score)at 1h,2h,4h,8h,12h,24h,48h after the operation of the two groups of patients,and record the occurrence of adverse reactions during the analgesia of the two groups.Results The VAS scores of the study group at 1h,2h,4h,8h,12h,24h,48h after operation were lower than those of the control group,but the difference was not statistically significant(P>0.05).There was no significant difference in Ramsay sedation score between the two groups of patients at 1h,2h,4h,8h,12h,24h,48h after operation(P>0.05).There were no adverse reactions such as respiratory depression in both groups,the total incidence of adverse reactions in the study group was 16.32%,which was significantly lower thanthe control group(38.78%)(P<0.05).Conclusion Fentanyl combined with ketorolac tromethamine intravenously controlled analgesia and fentanyl combined with dezocinehave similar effects.Both of which have good analgesic and sedative effects,but fentanyl combined with ketorolac tromethamine has fewer adverse reactions.
