某院乳腺癌患者心理困扰调查及其影响因素分析

Investigation on psychological distress of breast cancer patients in a hospital and its influencing factors analysis

目的采用美国国立综合癌症网络(NCCN)的心理困扰温度计量表[包括心理痛苦温度计(DT)及问题列表(PL)]等调查我国某院乳腺癌患者的心理困扰情况并分析其心理困扰产生的相关因素。方法以海南医学院第二附属医院乳甲外科2020年10月至2021年3月收治的乳腺癌患者为研究对象。采用DT、PL及一般资料调查表对患者进行横断面调查,并进行多因素logistic回归分析。结果共回收有效问卷258份。接受调查的乳腺癌患者均具有不同程度的心理困扰,显著困扰(DT评分大于或等于4分)的发生率为67.44%。患者的年龄、民族、家庭收入,疾病带来的经济负担,以及手术方式等因素与乳腺癌患者的心理困扰程度相关(P<0.05)。结论该院乳腺癌患者心理困扰发生率较高。临床工作中,护理人员不仅要做到对患者躯体疾病的护理,更要关注患者的心理状况,根据患者心理困扰产生的原因进行个体化心理护理,从而降低患者心理困扰的程度。

Objective To investigate the psychological distress of breast cancer patients in a hospital in China and analyze its related influencing factors by using the psychological distress thermometer scale[including distress thermometer(DT)and problem list(PL)]recommended by National Comprehensive Cancer Network(NCCN).Methods The breast cancer patients admitted to the Department of Thyroid and Breast Surgery of the Second Affiliated Hospital of Hainan Medical College from October 2020 to March 2021 were selected as the research objects.The patients were surveyed cross-sectional by using DT,PL and general information questionnaire,and multivariate logistic regression analysis was also performed.Results A total of 258 valid questionnaires were collected.All the patients had different degrees of psychological distress.The incidence of psychological distinct distress(scores of DT≥4)was 67.44%.The patients'age,nationality,family income,economic burden caused by disease and operation methods were related to the degree of psychological distress of patients with breast cancer(P<0.05).Conclusion The incidence of psychological distress of breast cancer patients is high in this hospital.In clinical work,nurses should not only care for the symptoms of patients,but also pay attention to the psychological status of patients,and carry out individualized psychological nursing according to the causes of psychological distress of patients,to reduce the degree of psychological distress of patients.
