摘要
目的:探究呼吸暂停综合征对高血压的影响。方法:选取2020年5月至2021年5月深圳华侨城医院收治的呼吸暂停综合征合并原发性高血压患者166例作为研究对象,收集患者基础资料,采用匹兹堡睡眠质量指数(PSQI)量表评价睡眠质量,并使用多导睡眠呼吸监测仪进行动态监测,同时动脉监测患者收缩压与舒张压。患者经鼻持续无创正压通气治疗,治疗后2个月评价与治疗前睡眠质量评分、呼吸暂停低通气指数(AHI)、最低血氧饱和度(LSaO_(2))、收缩压与舒张压。结果:治疗2个月后患者PSQI总分(4.45±0.87)分,显著低于治疗前PSQI总分(12.03±1.02)分,差异有统计学意义(P<0.05),且睡眠等级明显改善;治疗2个月后患者AHI(6.58±2.41)次/h、收缩压(121.56±10.14)mm Hg、舒张压(72.37±10.13)mm Hg,水平低于治疗前AHI(40.35±4.32)次/h、收缩压(150.47±14.28)mm Hg、舒张压(89.45±12.22)mm Hg水平。而治疗2个月后LSaO_(2)水平(86.59±10.36)%高于治疗前(72.68±10.12)%,差异有统计学意义(P<0.05)。结论:呼吸暂停综合征与高血压密切相关,通过持续无创正压通气改善患者睡眠呼吸暂停情况的同时有助于调控患者的血压指标。
Objective:To explore the influence of apnea syndrome on hypertension.Methods:A total of 166 patients with apnea syndrome complicated with essential hypertension admitted to Shenzhen OCT hospital from May 2020 to may 2021 were selected as the research objects.The basic data of the patients were collected.The Pittsburgh sleep quality index(PSQI)scale was used to evaluate the sleep quality,and the multi-channel sleep breathing monitor was used for dynamic monitoring.At the same time,the systolic and diastolic blood pressure of the patients were monitored by artery.The patients were treated with nasal continuous non-invasive positive pressure ventilation.Two months after treatment,the sleep quality score,apnea hypopnea index(AHI),minimum oxygen saturation(LSaO_(2)),systolic and diastolic blood pressure were evaluated and compared with those before treatment.Results:After 2 months of treatment,the total PSQI score(4.45±0.87)was significantly lower than that before treatment(12.03±1.02),the difference was statistically significant(P<0.05),and the sleep grade was significantly improved.After 2 months of treatment,the levels of AHI(6.58±2.41)times/h,systolic blood pressure(121.56±10.14)mm Hg and diastolic blood pressure(72.37±10.13)mm Hg were lower than those of AHI(40.35±4.32)times/h,systolic blood pressure(150.47±14.28)mm Hg and diastolic blood pressure(89.45±12.22)mm Hg before treatment.The LSaO_(2) level(86.59±10.36)%after 2 months of treatment was higher than that before treatment(72.68±10.12)%with significant difference(P<0.05).Conclusion:Apnea syndrome is closely related to hypertension.Continuous non-invasive positive pressure ventilation can improve patients′sleep apnea and help regulate blood pressure indicators.
