摘要
目的:探究糖尿病性视网膜病变早期诊断中运用OCTA的临床价值。方法:选择2019年6月-2021年6月某院收治的60例尚未出现视网膜病变的糖尿病患者作为本次的研究对象成立观察组(全部观察右眼),另招募同期60例健康者成立本次的对照组(全部观察右眼)。观察组和对照组均全部进行OCTA检查,通过对相关数据进行统计学分析,探究OCTA对糖尿病性视网膜病变早期诊断的价值。结果:观察组糖化血红蛋白、空腹血糖显著高于对照组,差异有统计学意义(P<0.05);观察组黄斑区浅层毛细血管丛、深层毛细血管丛、中心凹无血管区旁宽300um区的毛细血管血流密度低于对照组,差异有统计学意义(P<0.05);观察组中心凹无血管区面积、周长、非圆度指数指标高于对照组,差异有统计学意义(P<0.05);HbA1c与SVC、DVC、FD300均有相关性(P<0.05);FPG与DVC有相关性(P<0.05),FPG与SVC、FD300相关性较差(P>0.05)。结论:糖尿病视网膜病变早期OCTA检查可见患者眼底黄斑区血流密度降低,中心凹无血管区面积、周长、非圆度指数升高,可以对患者病情进行提示,具有显著的临床应用价值,建议进行推广应用。
Objective:To explore the clinical value of OCTA in the early diagnosis of diabetic retinopathy.Methods:From June 2019 to June 2021,60 diabetic patients without retinopathy in a hospital were selected as the research objects to set up the observation group(all observed in the right eye),and 60 healthy people were recruited to set up the control group(all observed in the right eye).The observation group and the control group were all examined by OCTA,and the value of OCTA in the early diagnosis of diabetic retinopathy was explored by statistical analysis of relevant data.Results:The glycosylated hemoglobin and fasting blood glucose in the observation group were significantly higher than those in the control group,and the difference was statistically significant(P<0.05).The capillary blood flow density of superficial capillary plexus,deep capillary plexus and 300um wide area beside the avascular area of fovea in macular region in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).The area,perimeter and non-roundness index of the avascular area of fovea in observation group were higher than those in control group,and the difference was statistically significant(P<0.05).HbAlc was correlated with SVC,DVC and FD300(P<0.05).FPG was correlated with DVC(P<0.05),and FPG was poorly correlated with SVC and FD300(P>0.05).Conclusion:In the early stage of diabetic retinopathy,OCTA examination shows decreased blood flow density in the macular area of the fundus,increased area,perimeter and non-roundness index in the avascular area of fovea,which can prompt the patient's condition,has significant clinical application value,and is recommended for promotion and application.
