老年髋部骨折术后肺部感染影响因素的性别差异

Gender diff erences in infl uencing factors of pulmonary infection after hip fracture surgery in the elderly

目的探讨老年髋部骨折术后肺部感染的影响因素是否存在性别差异。方法选择深圳市某三级甲等医院2012年1月—2020年12月收治的1409例老年髋部骨折患者为研究对象,收集既往文献中确定的术后肺部感染的影响因素,采用二元Logistic回归模型,分析男性和女性患者术后肺部感染的影响因素。结果本研究中老年髋部骨折术后肺部感染的发生率为6.03%,男性高于女性(P<0.05)。多因素Logistic回归分析显示,男性特有的术后肺部感染主要影响因素是低蛋白血症(OR=2.560),女性特有的术后肺部感染主要影响因素是吸烟史(OR=9.835)、糖尿病(OR=2.079)和脑卒中(OR=2.644)。结论老年髋部骨折术后肺部感染的发生率男性高于女性,不同性别的患者存在特有的术后肺部感染的影响因素,护理人员针对主要影响因素制订相应护理干预对策和管理策略。

Objective To explore whether there were gender diff erences in the infl uencing factors of pulmonary infection after hip fracture surgery in elderly patients.Methods A total of 1409 elderly patients with hip fracture admitted to a tertiary hospital in Shenzhen from January 2012 to December 2020 were selected as the study objects.The infl uencing factors of postoperative pulmonary infection identified in previous literature were collected,and binary Logistic regression model was used to analyze the infl uencing factors of postoperative pulmonary infection in male and female patients.Results The incidence of pulmonary infection after hip fracture in the elderly in this study was 6.03%,which was higher in men than in women(P<0.05).Multivariate Logistic regression analysis showed that the main influencing factor of male-specific postoperative pulmonary infection was hypoproteinemia(OR=2.560),and the main influencing factors of female-specific postoperative pulmonary infection were smoking history(OR=9.835),diabetes mellitus(OR=2.079)and stroke(OR=2.644).Conclusion The incidence of pulmonary infection after hip fracture operation in elderly men was higher than that in women.Patients of diff erent genders had unique infl uencing factors of postoperative pulmonary infection.Nursing staff formulate corresponding nursing intervention measures and management strategies according to the main infl uencing factors.
