集束化护理在放射性口腔炎患者中的应用

Application of cluster nursing in patients with radiation stomatitis

目的探究集束化护理在放射性口腔炎患者临床护理中的应用价值。方法选择2020年2月—2021年5月医院诊断为放射性口腔炎的头颈部肿瘤患者150例为研究对象,按照组间基本特征具有可比性的原则分为观察组80例与对照组70例,对照组采用常规护理,观察组采用集束化护理,比较两组患者干预前后口腔黏膜损伤程度、负性情绪、疼痛和生活质量变化情况。结果观察组口腔黏膜损伤程度低于对照组,组间比较差异有统计学意义(P<0.05)。干预前,两组负性情绪评分比较差异无统计学意义(P>0.05);干预后,观察组负性情绪评分低于对照组,组间比较差异有统计学意义(P<0.05)。干预前,两组疼痛评分比较差异无统计学意义(P>0.05);干预后3 d观察组疼痛评分低于对照组,组间比较差异有统计学意义(P<0.05)。干预前,两组生活质量各维度评分比较差异无统计学意义(P>0.05);干预后,观察组生活质量各维度评分高于对照组,组间比较差异有统计学意义(P<0.05)。结论头颈部肿瘤患者在出现放射性口腔炎后接受集束化护理,可缓解口腔黏膜损伤程度,调节负性情绪,降低疼痛程度,进一步提高患者生活质量。

Objective To explore the application value of cluster nursing in clinical nursing of patients with radiation stomatitis.Methods A total of 150 patients with head and neck tumors who were diagnosed with radiation stomatitis in the hospital from February 2020 to May 2021 were selected as the study subjects.According to the principle of comparability of basic characteristics between groups,they were divided into an observation group of 80 cases and a control group of 70 cases.The control group received routine nursing,and the observation group received cluster nursing.The changes in the degree of oral mucosal injury,negative emotion,pain,and quality of life were compared between the two groups before and after the intervention.Results The degree of oral mucosal injury in the observation group was lower than that in the control group,and the diff erence between the groups was statistically significant(P<0.05).Before the intervention,there was no significant difference in the score of negative emotion between the two groups(P>0.05).After the intervention,the score of negative emotion in the observation group was lower than that in the control group,and the diff erence between the groups was statistically signifi cant(P<0.05).Before the intervention,there was no signifi cant diff erence in the pain score between the two groups(P>0.05).The pain score of the observation group was lower than that of the control group 3 days after the intervention,and the diff erence between the groups was statistically signifi cant(P<0.05).Before the intervention,there was no signifi cant diff erence in the each dimension scores of quality of life between the two groups(P>0.05).After the intervention,the each dimension scores of quality of life in the observation group were higher than those in the control group,and the differences between the groups were statistically signifi cant(P<0.05).Conclusion The application of cluster nursing in head-neck tumor patients after receiving radiation stomatitis can relieve the degree of oral mucosal injury,regulate negative emotions,reduce the degree of pain,and further improve the living quality of patients.
