绝经后骨质疏松病人CCL3和IGFBP-3表达与骨密度的相关性 被引量:3

CORRELATION OF THE EXPRESSION OF CHEMOKINE LIGAND 3 AND INSULIN-LIKE GROWTH FACTOR BINDING PROTEIN-3 WITH BONE MINERAL DENSITY IN POSTMENOPAUSAL OSTEOPOROSIS

摘要 目的研究血清趋化因子配体3(CCL3)和胰岛素样生长因子结合蛋白-3(IGFBP-3)表达与绝经后骨质疏松病人股骨颈和腰椎骨密度(BMD)的相关性。方法以我院2017年1月—2021年1月诊断并治疗的绝经期骨质疏松病人120例为观察组,根据BMD检测结果分为骨量减少组59例、轻度骨质疏松组33例、重度骨质疏松组28例,另选取同期进行健康体检的绝经期非骨质疏松妇女为对照组,比较两组以及不同严重程度骨质疏松病人的血清CCL3、IGFBP-3表达及股骨颈和腰椎BMD的差异,并分析其相关性。结果观察组CCL3表达显著高于对照组,IGFBP-3表达和腰椎、左股骨颈及右股骨颈BMD水平均显著低于对照组(t=6.341~72.377,P<0.001)。不同程度骨质疏松病人血清CCL3与IGFBP-3表达以及腰椎、左股骨颈和右股骨颈BMD水平比较,差异有统计学意义(F=7.288~26.761,P<0.001)。随着病程进展,血清CCL3表达显著升高,IGFBP-3表达以及腰椎、左股骨颈和右股骨颈BMD显著降低。<60岁组血清CCL3表达显著低于≥60岁组,IGFBP-3表达显著高于≥60岁组,差异具有统计学意义(t=4.935、11.623,P<0.001)。骨质疏松病人CCL3表达与腰椎、左股骨颈和右股骨颈BMD均呈负相关(r=-0.647~-0.339,P<0.001),而IGFBP-3表达则与其均呈正相关(r=0.552~0.851,P<0.001)。结论血清CCL3、IGFBP-3表达与绝经后骨质疏松病人股骨颈和腰椎BMD有相关性。 Objective To investigate the correlation of the expression of serum chemokine ligand 3(CCL3)and insulin-like growth factor binding protein-3(IGFBP-3)with the bone mineral density(BMD)of femoral neck and lumbar vertebra in patients with postmenopausal osteoporosis.Methods A total of 120 patients with postmenopausal osteoporosis who were diagnosed and treated in our hospital from January 2017 to January 2021 were enrolled as observation group,and according to the results of BMD,they were divided into bone mass reduction group with 59 patients,mild osteoporosis group with 33 patients,and severe osteoporosis group with 28 patients;in addition,the postmenopausal women without osteoporosis who underwent physical examination during the same period of time were enrolled as control group.The expression of serum CCL3 and IGFBP-3 and BMD of femoral neck and lumbar vertebra were compared between the observation group and the control group and between the patients with different severities of osteoporosis,and a correlation analysis was also performed.Results Compared with the control group,the observation group had a significantly higher expression level of CCL3 and significantly lower expression level of IGFBP-3 and BMD levels of lumbar vertebra,left femoral neck,and right femoral neck(t=6.341-72.377,P<0.001).There were signifi-cant differences in the expression of serum CCL3 and IGFBP-3 and the BMD levels of lumbar vertebra,left femoral neck,and right femoral neck between the patients with different severities of osteoporosis(F=7.288-26.761,P<0.001).With the progression of the disease,there was a significant increase in the expression of serum CCL3 and significant reductions in the BMD levels of lumbar vertebra,left femoral neck,and right femoral neck.Compared with the≥60 years group,the<60 years group had a significantly lower expression level of serum CCL3 and a significantly higher expression level of IGFBP-3(t=4.935,11.623;P<0.001).In the patients with osteoporosis,the expression of CCL3 was negatively correlated with the BMD of lumbar vertebra,left femoral neck,and right femoral neck(r=-0.647 to-0.339,P<0.001),while the expression of IGFBP-3 was positively correlated with the BMD of lumbar vertebra,left femoral neck,and right femoral neck(r=0.552-0.851,P<0.001).Conclusion The expression levels of serum CCL3 and IGFBP-3 are correlated with the BMD of femoral neck and lumbar vertebra in patients with postmenopausal osteoporosis.
