药物联合心理剧治疗对女性抑郁症患者抑郁情绪、心理弹性和总体幸福感的作用

Effect of drugs combined with psychodrama therapy on emotion, resilienceand general well-being of female patients with major depressive disorder

目的:探讨抗抑郁药物联合易术心理剧疗法对女性抑郁症患者抑郁情绪、心理弹性和总体幸福感的作用。方法:采用随机对照研究,纳入观察组(n=24)与对照组(n=23),在一线抗抑郁药物治疗的基础上,观察组给予易术心理剧治疗,对照组予以一般健康教育。于干预前和经6个月干预后评定贝克抑郁量表(BDI)、心理弹性量表(CD-RISC)、总体幸福感量表(GWB)并采用独立样本t检验、重复测量方差分析比较其差异。结果:干预后两组患者之间的BDI、CD-RISC总分及其3个维度、GWB总分及其6个维度均存在干预*时间交互作用(F=9.061,5.991,5.070,5.413,5.157,7.534,4.998,4.091,5.339,4.361,8.122,4.606;P<0.05),两组BDI评分均有所下降(F=95.292,28.610;P<0.001),且观察组BDI评分低于对照组(F=10.073,P<0.05);观察组CD-RISC总评分及其各维度评分升高(F=27.925,22.783,22.001,31.889;P<0.001),对照组乐观维度评分升高(F=5.523,P<0.05),且观察组CD-RISC总分及其各维度评分高于对照组(F=10.360,10.870,6.544,8.686;P<0.05);观察组GWB总评分及其各维度评分均有升高(F=39.784,18.343,19.984,32.516,35.114,43.256,39.876;P<0.001),对照组GWB总分及其多维度(包括对生活的满足和兴趣、忧郁或愉快的心境、对情感和行为的控制、松弛与紧张维度)升高(F=7.975,5.517,8.285,6.004,10.103;P<0.05),且观察组GWB总分及其多维度(包括精力、对生活的满足和兴趣、忧郁或愉快的心境等)评分高于对照组(F=8.396,13.370,9.637,9.597;P<0.05),余量表分数无统计学差异(P>0.05)。结论:抗抑郁药物联合易术心理剧治疗较联合一般健康教育方案改善女性抑郁症患者抑郁情绪、心理弹性及总体幸福感的总体效果更好,药物联合易术心理剧方案具有临床应用价值。

Objective:To explore the effect of drugs combined with psychodrama therapy on the mood, resilience, and general well-being of female patients with major depressive disorder.Methods:A randomized controlled study was adopted, and the observation group(n=24)and the control group(n=23)were included.Based on first-line antidepressant treatment, the observation group was given Yi Shu psychodrama treatment.The control group was given general health education.The Beck Depression Inventory(BDI),Psychological Resilience Scale(CD-RISC),and General Well-Being Scale(GWB)were evaluated and compared before the intervention and at the end of the 6-month intervention by independent sample t-test and repeated measurement analysis of variance.Results:After intervention, there were intervention*time interaction results existence between the two groups in BDI score, CD-RISC’s total score and its three dimensions, GWB’s total score and its six dimensions(F=9.061,5.991,5.070,5.413,5.157,7.534,4.998,4.091,5.339,4.361,8.122,4.606;P<0.05).BDI score decreased in both groups(F=95.292,28.610;P<0.001),ang BDI score of the observation group was lower than that of the control group(F=10.073;P<0.05).In the observation group, CD-RISC’s total score and its all dimensions increased(F=27.925,22.783,22.001,31.889;P<0.001).In the control group, optimism dimension increased(F=5.523;P<0.05).The total score of CD-RISC and its various dimensions in the observation group were higher than those of the control group(F=10.360,10.870,6.544,8.686;P<0.05).GWB’s total score and its all dimensions in the observation group increased(F=39.784,18.343,19.984,32.516,35.114,43.256,39.876;P<0.001).And the total and several dimensions of GWB(including life satisfaction and support, depression or happy mood, control of emotions and behavior, relaxation and tension)in the control group increased(F=7.975,5.517,8.285,6.004,10.103;P<0.05).Besides, the total score of GWB and its several dimensions(including energy, life satisfaction and support, depression or happy mood)in the observation group were higher than those of the control group(F=8.396,13.370,9.637,9.597;P<0.05).The scores of other scales were not statistically different(P>0.05).Conclusion:Antidepressant drugs combined with Yi Shu psychodrama therapy is more effective than that combined with general health education programs in improving depression, resilience, and overall well-being in female depression patients, suggesting that the drug combined with Yi Shu psychodrama program has clinical application value.
