低位小切口手术治疗甲状腺良性结节的效果分析

Effect of low small incision thyroid surgery in the treatment of benign thyroid nodules

目的:分析低位小切口甲状腺手术治疗甲状腺良性结节的效果。方法:选取2019年6月—2020年12月在重庆市渝北区人民医院接受甲状腺手术治疗的100例甲状腺良性结节患者,采用随机数字表法将患者分为常规组与观察组各50例。常规组采取传统甲状腺手术进行治疗,观察组采取小切口甲状腺手术进行治疗,比较两组手术指标、并发症发生率、生活质量评分(SF-36)、术后疼痛视觉模拟评分(VAS)及治疗疗效。结果:观察组手术时间、住院时间、切口长度均短于常规组,术中出血量少于常规组,差异有统计学意义(P<0.05)。观察组并发症发生率(10.00%)低于常规组(32.00%),差异有统计学意义(P<0.05)。观察组治疗后VAS评分低于常规组,差异有统计学意义(P<0.05)。术后观察组SF-36生理功能、生理职能、精神健康、躯体疼痛、生命活力评分均高于常规组,差异有统计学意义(P<0.05);两组SF-36社会功能、情感职能评分比较,差异无统计学意义(P>0.05)。观察组治疗总有效率为96.00%高于常规组的82.00%,差异有统计学意义(P<0.05)。结论:低位小切口甲状腺手术切口长度较短,可有效缩短手术、住院时间,使术中出血量减少及术后并发症总发生率降低,缓解患者疼痛症状,提高患者生活质量。

Objective To observe and analyze the effect of low small incision thyroid surgery in the treatment of benign thyroid nodules.Methods 100 patients who underwent thyroid surgery in Yubei District People’s Hospital of Chongqing from June 2019 to December 2020 were selected as the research object.The patients were randomly divided into routine group and observation group,with 50 cases in each group.The conventional group was treated with traditional thyroid surgery,and the observation group was treated with small incision thyroid surgery.The general conditions,complication rate,quality of life score(SF-36)and postoperative pain visual analog score(VAS)were compared between the two groups.Results The operation,hospital stay and incision length in the observation group were shorter,and the amount of intraoperative bleeding in the observation group was significantly lower than that in the control group(P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group P.The VAS score of the observation group was significantly lower than that of the control group(P<0.05).The scores of SF-36 physiological function,physiological function,mental health,physical pain and vitality in the observation group were significantly higher than those in the control group(P<0.05);There was no significant difference in the scores of SF-36 social function and emotional function between the two groups(P<0.05).The total effective rate of the observation group was 96.00%,which was higher than 82.00%of the routine group(P<0.05).Conclusion The incision length of low small incision thyroid surgery is short,which can effectively shorten the operation and hospital stay,reduce the amount of intraoperative bleeding and postoperative complications,alleviate the pain symptoms and improve the quality of life of patients.
