柳州市铁路职工2型糖尿病合并糖尿病周围神经病变相关及其相关因素调查分析

Analysis of related factors of diabetic peripheral neuropathy in type 2 diabetes cases in Liuzhou railway workers

目的:通过对柳州市铁路职工2型糖尿病(T2DM)合并糖尿性周围神经病变(DPN)的调查,分析相关危险因素,为该人群DPN的防治提供临床依据。方法:调查2019年9月—2021年5月306例柳州市铁路T2DM职工,根据患者是否合并DPN将其分为DPN组和非DPN组,分析该人群合并DPN情况及相关的危险因素。结果:306例T2DM患者合并DPN205例,DPN的合并率为67%;DPN组血糖达标率(29.76%)低于非DPN组(64.36%),差异有统计学意义(P<0.05);DPN组糖尿病肾病合并率(61.95%)高于非DPN组(24.75%),差异有统计学意义(P<0.05)。通过Logistic回归分析,患者病程、吸烟、饮酒、睡眠不足、家属未参与患者管理、血糖未达标、非胰岛素降糖为合并DPN的危险因素,差异有统计学意义(P<0.05)。结论:柳州市铁路职工T2DM患者合并DPN率较高,根据T2DM患者合并DPN相关危险因素,应加强健康教育、优化降糖方案、鼓励家属参与患者管理等,以降低合并DPN的发生率及提高患者生存质量。

Objective To investigate diabetic peripheral neuropathy(DPN)in type 2 diabetes mellitus(T2DM)of Liuzhou railway workers and analyze the related risk factors,so as to provide a clinical basis for prevention and treatment of DPN in this population.Methods 306 railway T2DM workers in Liuzhou city from September 2019 to May 2021 were investigated.They were divided into DPN group and non DPN group.DPN and related risk factors were studied.Results The prevalence of DPN was 67%;The blood glucose standard rate in DPN group was lower than that in non DPN group(P<0.05);The incidence of diabetic nephropathy in group DPN was higher than that in non DPN group,and the difference was statistically significant(P<0.05).The course of disease,smoking,drinking,lack of sleep,family members not participating in patient management,blood glucose not reaching the standard and non insulin hypoglycemic were the risk factors of DPN,and the difference was statistically significant(P<0.05).Conclusion The prevalence of DPN among railway T2DM workers in Liuzhou is high.According to the risk factors related to DPN,strengthen special education,optimize hypoglycemic program,and encourage family members to participate in patient management,so as to reduce the prevalence rate of DPN and improve the quality of life of patients.
