妊娠期感染单核李斯特菌的2例报告及文献回顾

Analysis on Two Cases of Gestation Period With Listeria Monocytogenes Infection and Literature Review

目的分析孕妇感染单核细胞增生李斯特菌(Listeria monocytogenes,LM)的临床特点和预后,为临床提供相应指导。方法选择2例2021年8月和2022年1月感染李斯特菌病的孕妇,对其临床病例资料给予分析,包括孕妇的一般情况、临床症状、抗菌药物治疗经过和护理管理方面,以及胎儿的结局转归。结果2例孕妇感染LM发生在妊娠晚期,孕周为33~37周,住院时均发热,其中一例已产生宫缩;1例顺产1活婴,1例剖宫产1死婴。结论孕妇感染LM的临床表现不典型,伴有发热,表现为流感样症状,胎动异常。该病菌感染对孕妇危害不大但易造成胎儿死亡,主要采用抗生素药物治疗,产科医护工作者必须对感染李斯特菌的孕妇提高警惕,做到早发现、早诊断、早治疗,从而改善母婴结局。

Objective To investigate the clinical features and pregnancy outcomes of women with Listeria monocytogenes(LM)infection during pregnancy,in order to provide clinical guidance.Methods Two cases of pregnancy with LM infection from August 2021 to January 2022 were selected as research subjects.Their clinical data were reviewed retrospectively,including general conditions,clinical symptoms,antibiotics treatment,nursing management of pregnancy with LM infection,and general conditions of fetus or newborns.Results All the two pregnant women were infected in the third trimester of pregnancy,and the gestational period was 33~37 weeks.Two cases had a recent history of fever before admission,one of them was admitted due to abdominal pain.One was born alive through natural labor and one was occurred stillbirth by cesarean section.Conclusion The clinical features of pregnant women with LM infection during pregnancy are mainly nonspecific,often combined with fever,manifested as flulike symptoms and abnormal fetal movement.L M infection does little harm to pregnant women,but it is easy to cause fetal death.Antibacterial drugs are mainly used for treatment.So obstetric department worker should improve vigilance for pregnant women infected LM,so as to achieve early detection,early diagnosis,and early treatment,and improve the outcomes of maternal and neonatal.
