磁共振NODDI在帕金森病黑质致密部微观改变的临床应用

摘要 目的评估磁共振神经突方向离散度与密度成像(NODDI)对帕金森病患者黑质致密部(SNPC)变化的定量诊断价值。方法选取我院帕金森病(PD)患者41例(PD组)和年龄与性别匹配的正常志愿者22例(对照组)进行磁共振平扫和NODDI成像,量化分析PD患者黑质致密部(SNPC)的变化,在组间比较黑质致密部(SNPC)的细胞内体积分数(VIC),方向分散指数(ODI)和各向同性体积分数(VISO)。多因素Logistic回归分析确定PD的独立预测因子。结果PD患者SNPC中的VIC、ODI明显低于对照组(P<0.05),多因素Logistic分析显示,PD患者SNPC中的VIC值的变化与PD的相关性差异有统计学意义(P<0.05),SNPC中的VIC值是PD的独立预测因子。在ROC分析中,对侧SNPC中的VIC值显示出最佳的诊断性能。结论NODDI可以反映PD患者SNPC的微观变化,其参数值ODI、VIC值可以作为PD患者的评估指标,可以通过NODDI技术对PD患者做出定量诊断。 Objective To explore the clinical application of MR Neurite orientation dispersion and density imaging(NODDI)in patients with Parkinson's disease(PD).Methods 41 patients with PD and 22 age and sex matched normal volunteers were collected as control group to perform plain MRI and NODDI imaging.We uantitatively analyzed the changes of(SNPC)in the dense part of substantia nigra in patients with PD.The intracellular volume fraction(VIC),directional dispersion index(ODI)and isotropic volume fraction(VISO)of the dense part of the substantia nigra(SNPC)were compared between the two groups.Multivariate logistic regression analysis was used to determine the independent predictors of PD.Results The levels of VIC and ODI in SNPC of patients with PD were significantly lower than those in healthy controls.Multivariate Logistic analysis showed that the change of VIC in SNPC of patients with Parkinson's disease was significantly correlated with PD(P<0.05),SNPC was an independent predictor of PD.In the ROC analysis,the VIC in the contralateral SNPC showed the best diagnostic performance.Conclusion NODDI can reflect the microscopic changes of substantia nigra structure in patients with PD.The directional dispersion index(ODI)and intracellular volume fraction(VIC)can be used as evaluation indexes for PD patients and make diagnosis for PD patients.
