MRI延迟增强扫描在面神经炎中的临床应用价值探讨

目的探讨MRI在面神经炎延迟增强扫描中的临床价值,用来指导面神经炎MRI增强扫描序列的优化。方法选取我院确诊的面神经炎且未经过治疗患者共43例。所有患者均为右侧患病,均采用同一序列,同一扫描方法。将所有图像传送至Siemenssyngo MR B17工作站,分别测量同一患者双侧相同部位平扫,普通增强、延迟增强扫描面神经迷路段、鼓室段和乳突段的信号值(S),每个部位分别测量3次,取其平均值,利用(ΔS%)=(S增强-S平扫)/S平扫×100%作为信号强度的变化率,利用ΔS%来判断面神经的强化程度。结果普通增强组与延迟增强组之间差异有统计学意义(P<0.05);健侧与患侧比较,普通增强组:迷路段、鼓室段差异有统计学意义,乳突段差异无统计学意义;而延迟增强组,三段比较差异均有统计学意义,并且患侧的信号值均高于健侧。与临床结果对照,立即增强组检出率为89%,而通过延迟增强检出率达到96%。结论面神经炎在MRI增强扫描的应用中,延迟增强扫描是有必要的,值得推广应用。

Objective To explore the use of delayed contrast-enhanced scan in MR enhanced scanning of facial neuritis,to study its clinical value,and to guide the optimization of enhanced MR scanning sequence of facial neuritis.Methods A total of 43 patients with the facial neuritis diagnosed and untreated in our hospital were collected.All patients were on the right side of the disease,subjected to the same sequence,the same scanning method.All the images were transmitted to Siemenssyngo MR B17 workstation,and the signal values of the labyrinthine,tympanic and mastoid segments of the facial nerve were measured on both sides of the same patient.The signal values of each part were measured 3 times,and the average value was taken.(ΔS%)=(S enhancement-S mask)/S mask×100%was used as the signal change rate,andΔS%was used to judge the degree of facial nerve enhancement.Results There was statistical significance between the ordinary enhancement group and the delayed enhancement group(P<0.05).Compared with the affected side,there was statistical significance in the labyrinth segment and tympanic segment,but not in the mastoid segment in the common enhancement group,while in the delayed enhancement group,the signal values of the three segments were statistically significant,and the signal values of the affected side were higher than those of the healthy side.Compared with the clinical results,the detection rate of the immediate enhancement group was 89%,while the detection rate of delayed enhancement reached 96%.Conclusion Delayed contrast-enhanced scanning is necessary in the application of facial neuritis in contrast-enhanced MRI,and it is worth popularizing.
