糖尿病肾病血液透析患者透析过程中低血糖发生率及影响因素调查

Investigation on the Incidence and Influencing Factors of Hypoglycemia during Hemodialysis in Diabetic Nephropathy Patients

目的浅析糖尿病肾病血液透析患者透析过程中低血糖发生风险与影响因素,以便提升临床治疗安全性。方法选择2020年1月—2021年1月该院收治的128例糖尿病肾病血液透析患者作为研究对象,均接受常规治疗与血糖控制治疗,分析患者于治疗后1个月中12次血液透析过程中的血糖指标,研究低血糖发生状况,分析引发低血糖的影响因素。结果研究开始后1个月12次血液透析过程中共检测128例患者血糖1536次,低血糖发生率为51.56%(66/128)。多因素Logistic回归分析显示,发现年龄、低密度脂蛋白(LDL)、尿酸(UA)、用药依从性、照护能力、自我管理能力均为低血糖的影响因素(P<0.05)。结论年龄、LDL、UA、用药依从性、照护能力、自我管理能力均是糖尿病血液透析患者透析过程中低血糖发生的影响因素,临床需针对该类患者给予关注,保障治疗的安全性。

Objective To analyze the risk and influencing factors of hypoglycemia in hemodialysis patients with diabetic nephropathy in order to improve the safety of clinical treatment.Methods From January 2020 to January 2021,128 diabetic nephropathy hemodialysis patients treated in the hospital were selected as the research objects.All received conventional treatment and blood sugar control treatment.The blood glucose indexes of the patients during 12 hemodialysis sessions in one month after treatment were analyzed to study the occurrence of hypoglycemia.To analyze the influencing factors of hypoglycemia.Results A total of 1536 blood sugars were detected in 128 patients during 12 hemodialysis sessions one month after the start of the study,and the incidence of hypoglycemia was 51.56%(66/128).Multivariate Logistic regression analysis showed that age,low density lipoprotein(LDL),uric acid(UA),medication compliance,nursing ability,and self-management ability were all influencing factors hypoglycemia(P<0.05).Conclusion Age,LDL,UA,medication compliance,nursing ability and self-management ability are all influencing factors of hypoglycemia during dialysis in diabetic hemodialysis patients.Clinical attention should be paid to such patients to ensure the safety of treatment.
