甲状腺癌术后心理弹性水平现况、影响因素及护理对策研究

Study on status quo,influencing factors and nursing countermeasures of postoperative psychological elasticity of patients with thyroid cancer

目的:分析甲状腺癌患者术后心理弹性水平及相关影响因素,并总结其护理对策。方法:分析2019-06~2020-12我院收治的116例甲状腺癌患者各项一般资料及临床病历记录,采用中文版心理弹性水平量表(CDRISC)评价其术后心理弹性水平,并采经多因素Logistic回归分析相关影响因素,总结护理对策。结果:116例患者术后心理弹性平均得分(56.83±7.69)分,相较于国内常模(65.40±13.90)分较低(P<0.05)。单因素分析可见病程、文化程度、性别、肿瘤分期、应对方式与心理弹性水平存在一定相关性(P<0.05)进一步行经多因素Logistic回归分析可见,Ⅲ~Ⅳ期肿瘤、消极应对方式、女性、病程≥1年是甲状腺癌患者术后心理弹性水平的独立危险因素(P<0.05)。结论:甲状腺癌患者术后心理弹性水平尚处于较低水平,且与其肿瘤分期、应对方式、性别、病程等因素相关,尽早预防并给予患者综合护理干预有助于提高其心理弹力,从而改善疾病预后效果。

Objective:To analyze the level of postoperative psychological resilience of patients with thyroid cancer and the related influencing factors,and summarize the nursing countermeasures.Methods:The general data and clinical medical records of 116 patients with thyroid cancer who were admitted to our hospital from june 2019 to december 2020 were retrospectively analyzed.the postoperative psychological resilience level was evaluated using the chinese version of psychological resilience scale(CDRISC),and the related influencing factors were analyzed by multivariate Logistic regression,to summarize the nursing countermeasures.Results:The average score of postoperative resilience of 116 patients was(56.83±7.69),which was lower than the national norm(65.40±13.90)(P<0.05).Univariate analysis showed that there was a certain correlation between the course of disease,educational level,gender,tumor stage,and coping style and psychological resilience(P<0.05).Further multivariate Logistic regression analysis showed that stage III–IV tumors,negative coping styles,female,and disease course≥1 year were the independent risk factors for postoperative psychological resilience of patients with thyroid cancer(P<0.05).Conclusion:The postoperative psychological resilience of patients with thyroid cancer is still in a low level,which is closely related to such factors as tumor stage,coping style,gender,and course of disease.Early prevention and comprehensive nursing countermeasures can improve the psychological resilience,and thus improve the prognosis of the disease.
