摘要
目的探讨IDH不同突变亚型及伴发基因突变对急性前体B淋巴细胞白血病患儿的预后意义。方法选取收治急性前体B淋巴细胞白血病患儿130例,检测IDH不同突变亚型及伴发基因突变,分析其对急性前体B淋巴细胞白血病患儿预后的影响。结果130例患者中24例(18.46%)检出IDH突变,IDH1突变10例(7.69%),IDH2突变14例(10.77%)。IDH1、IDH2突变与患儿年龄、性别、WBC水平、染色体核型无相关性(P>0.05)。IDH1突变型急性B淋巴细胞白血病患儿的复发率显著高于野生型(P<0.05),IDH2突变型与野生型急性B淋巴细胞白血病患儿的复发率差异无统计学意义(P>0.05)。IDH1突变型急性B淋巴细胞白血病患儿的总生存率显著低于野生型(P<0.05),IDH2突变型与野生型急性B淋巴细胞白血病患儿的总生存率差异无统计学意义(P>0.05)。结论在急性前体B淋巴细胞白血病患儿中,不同IDH基因型突变显示出不同的预后情况,IDH1突变型患儿的复发率偏高以及生存率偏低。
Objective To investigate the prognostic significance of different IDH mutation subtypes and concomitant gene mutations in children with acute precursor B lymphocytic leukemia.Methods A total of 130 children with acute precursor B lymphocytic leukemia were selected.Different mutation subtypes and concomitant gene mutations of IDH were detected,and their effects on the prognosis of children with acute precursor B lymphocytic leukemia were analyzed.Results Among the 130 patients,24(18.46%)had IDH mutations,10(7.69%)had IDH1 mutations,and 14(10.77%)had IDH2 mutations.There was no correlation between IDH1 and IDH2 mutations and age,gender,WBC level and karyotype(P>0.05).The recurrence rate of children with IDH1 mutant acute B lymphocytic leukemia was significantly higher than that of wild type(P<0.05),but there was no significant difference between the recurrence rate of IDH2 mutant and wild type acute B lymphocytic leukemia(P>0.05).The overall survival rate of children with IDH1 mutant acute B lymphocytic leukemia was significantly lower than that of wild type(P<0.05),and there was no significant difference in the overall survival rate between IDH2 mutant and wild type acute B lymphocytic leukemia(P>0.05).Conclusion In children with acute precursor B lymphocytic leukemia,different IDH genotype mutations show different prognosis.Children with IDH1 mutant have a higher recurrence rate and a lower survival rate.
