GBT指导下的定期维护对伴2型糖尿病的牙周炎治疗疗效的影响

The influence of regular maintenance under the guidance of GBT on the curative effect of periodontitis with type 2 diabetes

目的评价以菌斑为导向的牙周治疗方案(GBT)指导下的甘氨酸龈下喷砂在伴2型糖尿病的牙周炎进入牙周维护期的临床应用效果。方法选取伴2型糖尿病的牙周炎患者30例,基础治疗后按是否定期维护治疗筛选出15例不定期维护患者为A组、15例定期维护患者为B组,记录菌斑指数(plaque index,PLI)、探诊深度(probing depth,PD)、探诊出血阳性位点百分比[bleeding on probing,BOP(+)%]及临床附着丧失(clinical attachment loss,CAL),比较两组基础治疗前、治疗后1个月以及1年后最后一次就诊时各项牙周指标,同时计算两组失牙率。结果与基础治疗前相比,治疗后1个月和1年后两组各项指标均有显著改善(P<0.05);治疗1年后,B组与A组相比较,各项指标之间有显著性差异(P<0.05);两组患者1年后最后一次就诊时,A组失牙率高于B组,但没有统计学差异(P>0.05)。结论GBT指导下的定期维护能清除伴2型糖尿病牙周炎患者的龈下菌斑,并有效控制牙周炎症,值得推广。

Objective To evaluate the clinical effect of GBT-guided glycine subgingival sandblasting in periodontitis with type 2 diabetes.Methods Thirty patients with periodontitis with type 2 diabetes were selected.According to whether they were regularly maintained and treated after periodontal initial therapy,15 patients with non-regular maintenance were screened as group A and 15 patients with regular maintenance as group B.The plaque index(PLI),probing depth(PD),percentage of positive sites of probing bleeding(bleeding on probing,BOP(+)%)and clinical attachment loss(CAL)of the two groups were recorded and compared before basic treatment,one month after treatment and at the last visit.The tooth loss rate of the two groups was calculated.Results PLI,PD,BOP and CAL in the two groups were significantly improved at one month and one year after treatment(P<0.05).One year after treatment,there was a significant difference in the above indicators between the two groups(P<0.05).At the last visit,the rate of tooth loss in group A was higher than that in group B,but there was no statistical difference(P>0.05).Conclusions Regular maintenance under the guidance of GBT can remove the subgingival plaque of periodontitis patients with type 2 diabetes and effectively control periodontal inflammation.
