培元调神针灸法治疗卵巢早衰肾虚肝郁证的临床研究

Clinical study on Peiyuan Tiaoshen acupuncture in the treatment of premature ovarian failure with syndrome of kidney deficiency and liver depression

目的评估培元调神针灸法治疗卵巢早衰肾虚肝郁证的临床疗效。方法选择2018年12月—2020年12月在青岛市第八人民医院治疗的80例卵巢早衰肾虚肝郁证患者,按照就诊顺序以随机数字表法分为对照组及治疗组各40例。治疗组给予培元调神针灸法治疗,对照组予口服戊酸雌二醇片、黄体酮胶囊序贯疗法治疗,2组均以3个月为1个疗程,共治疗1个疗程。观察2组治疗前后血清黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E_(2))水平及窦卵泡计数(AFC)、国内改良Kupperman量表评分、焦虑自评量表(SAS)评分变化,评估2组临床疗效。结果2组治疗后血清FSH水平均明显降低(P均<0.05),E_(2)水平均明显升高(P均<0.05),且治疗组改善幅度优于对照组(P均<0.05);治疗组治疗后血清LH水平明显低于治疗前及对照组(P均<0.05),AFC明显多于治疗前及对照组(P均<0.05),对照组治疗后血清LH水平、AFC变化不明显(P均>0.05)。治疗后,2组改良Kupperman量表评分均明显降低(P均<0.05),且治疗组明显低于对照组(P<0.05);治疗组SAS评分明显低于治疗前及对照组(P均<0.05),对照组SAS评分无明显变化(P>0.05)。治疗组的总有效率为90.0%(2/40),明显高于对照组的65.0%(36/40),差异有统计学意义(P<0.05)。治疗组不良反应发生率为5.0%(2/40),明显低于对照组的17.5%(7/40),差异有统计学意义(P<0.05)。结论培元调神针灸法可明显调节卵巢早衰肾虚肝郁证患者的性激素水平,增加AFC,改善更年期症状及焦虑状态,且安全性较好。

Objective It is to evaluate the clinical efficacy of Peiyuan Tiaoshen acupuncture in the treatment of premature ovarian failure with syndrome of kidney deficiency and liver depression.Methods Eighty patients with premature ovarian failure with syndrome of kidney deficiency and liver depression who were treated in Qingdao Eighth People’s Hospital from December 2018 to December 2020 were selected and divided into control group and treatment group by random number table method according to the order of treatment.The treatment group was treated with Peiyuan Tiaoshen acupuncture and moxibustion,and the control group was treated with sequential therapy of estradiol valerate tablets and progesterone capsules by oral administration.3 months as a course of treatment,both groups were treated for a total of 1 course of treatment.The changes in the levels of serum luteinizing hormone(LH),follicle-stimulating hormone(FSH),estradiol(E_(2)),antral follicle count(AFC),and the scores of domestic modified Kupperman scale and self-rating anxiety scale(SAS)before and after treatment were observed in both groups.The clinical efficacy of the two groups was evaluated.Results After treatment,the levels of serum FSH were significantly decreased(both P<0.05),and the levels of E_(2) were significantly increased in both groups(both P<0.05),and the improvement in the treatment group was better than that in the control group(P<0.05).After treatment,the levels of serum LH were significantly lower while AFC in the treatment group was significantly higher than that before treatment and in the control group(P<0.05),there was no significant differences in the level of serum LH and AFC in the control group(all P>0.05).After treatment,the scores of modified Kupperman scale were significantly decreased in both groups(all P<0.05),and the treatment group was significantly lower than the control group(P<0.05);the SAS score in the treatment group was significantly lower than those before treatment and in the control group(P<0.05),and there was no significant change in the SAS score in the control group(P>0.05).The total effective rate in the treatment group was 90.0%(2/40),which was significantly higher than 65.0%(36/40)in the control group,the difference was statistically significant(P<0.05).The incidence of adverse reactions in the treatment group was 5.0%(2/40),which was significantly lower than 17.5%(7/40)in the control group,and the difference was statistically significant(P<0.05).Conclusion Peiyuan Tiaoshen acupuncture can significantly regulate the level of sex hormones,increase AFC,improve menopausal symptoms and anxiety with good safety in the treat of patients with premature ovarian failure with syndrome of kidney deficiency and liver depression.
