椎间融合术联合单边椎弓根钉棒内固定治疗腰椎间盘突出症的临床疗效观察

Clinical Efficacy of Interbody Fusion Combined with Unilateral Pedicle Screw Fixation for Lumbar Disc Herniation

目的:探讨椎间融合术联合单边椎弓根钉棒内固定治疗腰椎间盘突出症的临床疗效。方法:回顾2018年6月—2020年4月武陟新区医院收治的80例腰椎间盘突出症患者临床资料,根据治疗方案不同分为常规组与联合组。其中40例患者接受常规术式治疗,纳入常规组;40例患者接受椎间融合术联合单边椎弓根钉棒内固定治疗,纳入联合组。比较两组患者一般情况、生活质量评分、疼痛评分、功能障碍评分。结果:联合组患者手术时间、术中出血量、VAS评分、Oswestry评分少于常规组,差异有统计学意义(t=5.970、36.018、0.095、4.247、5.614、25.479,P<0.05)。联合组患者生活质量各维度生功能(PF)、生理职能(RP)、总体健康(GH)、活力(VT)、社会功能(SF)、情感职能(RE)及精神健康(MH)评分显著高于常规组,差异有统计学意义(t=0.012、0.264、0.000、0.010、0.046、0.030、0.359,P<0.05)。结论:椎间融合术联合单边椎弓根钉棒内固定治疗腰椎间盘突出症效果较佳,能显著改善患者术后疼痛,提高生活质量,缓解疼痛,可推荐使用。

Objective:To investigate the clinical efficacy of interbody fusion combined with unilateral pedicle screw fixation for lumbar disc herniation.Methods:The clinical data of 80 patients with lumbar intervertebral disc herniation admitted to the hospi⁃tal from June 2018 to April 2020 were reviewed,and they were divided into conventional group and combined group according to different treatment plans.Among them,40 patients received conventional surgical treatment and were included in the conventional group,and 40 patients received interbody fusion combined with unilateral pedicle screw-rod internal fixation and were included in the combined group.The general condition,quality of life score,pain score,and dysfunction score were compared between the two groups.Results:The operation time,intraoperative blood loss,VAS score and Oswestry score in the combined group were less than those in the conventional group,and the differences were statistically significant(t=5.970,36.018,0.095,4.247,5.614,25.479,P<0.05).The scores of physical functioning(PE),role physical(RP),general health(GH),vitality(VT),social functioning(SF),role emotional(RE),mental health(MH)in the combined group were significantly higher than those in the conventional group,and the difference was statistically significant(t=0.012,0.264,0.000,0.010,0.046,0.030,0.359,P<0.05).Conclusion:Interbody fusion combined with unilateral pedicle screw and rod internal fixation has a good effect in the treatment of lumbar disc herniation,and can significantly improve postoperative pain,improve the quality of life,and relieve pain.It can be recommended.
