肝细胞癌自发性破裂出血治疗的研究进展

Research progress of spontaneous ruptured hepatocellular carcinoma

肝细胞癌自发性破裂出血发病急,进展迅速,发病机制不明确,且患者肝功能背景复杂,个体之间异质性较大,不同的处理方式所取得的生存获益也不一致,对临床决策的选择造成了很大的困扰,因此寻找一些指标或模型来预测肝细胞癌自发性破裂患者的治疗预期意义重大。本文基于现有的临床证据,从肝细胞癌自发性破裂的机制阐述了高破裂倾向型肝细胞癌的手术时机,对肝细胞癌自发性破裂后不同处理方式所取得的近远期生存情况和多个模型对肝细胞癌治疗方式的预测效果,以及肝细胞自发性破裂幸存者的后续治疗策略进行综述。

Spontaneously ruptured hepatocellular carcinoma(rHCC)with hemorrhage is characterized by rapid onset and progression,the pathogenesis is not clear,and liver background in rHCC patients was complicated.There is large heterogeneity between different ways of handling the survival benefit obtained,the choice of clinical decision caused great distress,so looking for some indicators or model to predict the treatment of patients with rHCC forecast is of great significance.This article is based on the existing clinical evidence,this article will review the research progress of rHCC with hemorrhage:From the rupture mechanism of hepatocellular carcinoma(HCC)are expounded of high burst tendency HCC of the operation time.Different treatments of short-and long-term survival benefit with multiple model prediction effect for the treatment of rHCC.Finally we explored the treatment of rHCC survivors.
