脑卒中患者血浆ICAM-1和脑钠肽水平与神经功能缺损的关联性研究 被引量:2

Correlation between plasma levels of ICAM-1,brain natriuretic peptide and neurological deficits in stroke patients

摘要 目的探讨脑卒中患者血浆细胞间黏附分子-1(ICAM-1)和脑钠肽水平与神经功能缺损的关联性。方法回顾性选取2020年1月至2021年10月于粤北人民医院住院诊断为脑卒中的患者121例作为脑卒中组,同期选择在粤北人民医院体检的健康人群121名作为对照组。检测两组血浆ICAM-1和脑钠肽水平,调查两组的美国国立卫生研究院卒中量表(NIHSS)、简易精神状态量表(MMSE)、蒙特利尔认知评估量表(MoCA)评分,判断神经功能缺损发生情况并进行相关性分析。结果脑卒中组的血浆ICAM-1和脑钠肽含量为(398.26±21.42)ng/L、(145.67±15.35)pg/mL,高于对照组[(222.19±11.37)ng/L、(21.11±3.29)pg/mL],差异有统计学意义(P<0.05)。脑卒中组的MMSE、MoCA评分为(15.02±1.11)、(14.82±2.10)分,低于对照组[(28.14±0.24)、(29.14±0.33)分],NIHSS评分(19.28±1.11)分高于对照组[(5.29±0.32)分],差异有统计学意义(P<0.05)。在脑卒中121例患者中,判断为神经功能缺损36例,占比29.75%。直线相关分析显示,MMSE、MoCA、NIHSS评分与血浆ICAM-1、脑钠肽含量存在相关性(P<0.05);多元线性回归分析显示,血浆ICAM-1、脑钠肽含量为导致神经功能缺损的重要因素(P<0.05)。结论脑卒中患者神经功能缺损发生率较高,多伴随有血浆ICAM-1、脑钠肽的高表达,且血浆ICAM-1、脑钠肽含量为导致神经功能缺损的重要因素。 Objective To investigate the correlation between plasma intercellular adhesion molecule-1(ICAM-1)and brain natriuretic peptide(BNP)levels and neurological deficits in stroke patients.Methods A total of 121 cases of patients with stroke who were hospitalized in North Guangdong People's Hospital from January 2020 to October 2021 were retrospectively selected as the stroke group,and the other 121 cases of healthy people who underwent physical examination in North Guangdong People's Hospital during the same period were selected as the control group.The plasma ICAM-1,BNP levels in the two groups were detected.The National Institute of Health Stroke Scale(NIHSS),Mini-mental State Examination(MMSE),Montreal Cognitive Assessment Scale(MoCA)scores were investigated in the two groups.And were to judge the occurrence of neurological deficits and conduct correlation analysis.Results The plasma levels of ICAM-1 and BNP in the stroke group were(398.26±21.42)ng/L,(145.67±15.35)pg/mL,which were higher than those in the control group[(222.19±11.37)ng/L,(21.11±3.29)pg/mL],the differences were statistically significant(P<0.05).The MMSE and MoCA scores in the stroke group were(15.02±1.11),(14.82±2.10)points,which were lower than those in the control group[(28.14±0.24),(29.14±0.33)points],and the NIHSS scores were(19.28±1.11)points,which were lower than those in the control group[(5.29±0.32)points],the differences were statistically significant(P<0.05).In the 121 stroke patients,there were 36 patients were judged to be neurological deficit,accounted for 29.8%;linear correlation analysis showed that MMSE,MoCA,NIHSS scores were correlated with plasma ICAM-1 and BNP levels(P<0.05);multivariate linear Regression analysis showed that plasma levels of ICAM-1 and BNP were important factors leaded to neurological deficits(P<0.05).Conclusion Stroke patients have a high incidence of neurological deficits,which are often accompanied by high expression of plasma ICAM-1 and BNP.And plasma ICAM-1 and BNP levels are important factors leading to neurological deficits.
