老年2型糖尿病中性粒细胞与淋巴细胞比值与冠心病相关性的研究 被引量:7

The relationship between neutrophil to lymphocyte ratio and coronary artery disease in elderly patients with type 2 diabetes mellitus

摘要 目的:越来越多的研究表明,NLR可能对心血管疾病存在一定的影响,本研究基于老年2型糖尿病(T2DM)人群,旨在探讨NLR与冠心病的相关性。方法:本研究纳入北京市安贞医院2017年1月至6月,住院行冠状动脉造影的年龄> 60岁的患者共4 832例,其中有1 566例T2DM患者。通过NLR值组间比较、NLR与在两组间与基线指标的相关性以及单多因素Logistics回归分析,NLR在T2DM人群中与冠心病的相关性,同时也通过限制性立方样条探索NLR与冠心病的线性关系。结果:NLR在冠心病合并T2DM老年人群中均高于对照组,同时与传统危险因素包括性别、高血压、年龄、吸烟、空腹血糖、肌酐均呈显著正相关。单因素和多因素Logistics回归分析提示,NLR与冠心病呈现显著相关性(多因素回归模型3:总人群:OR=1.089,95%CI:1.014~1.170,P=0.019;T2DM人群:OR=1.121,95%CI:1.003~1.253,P=0.045)。结论:NLR作为临床易获得的生物标志物,其与老年冠心病合并T2DM人群相关。 Objective: There is increasing evidence that the NLR may be a useful marker of cardiovascular disease. This study was conducted in elderly patients with type 2 diabetes mellitus(T2DM)to investigate the correlation between the NLR and coronary artery disease. Methods: A total of 4832 patients over 60 years old who underwent coronary angiography in Beijing Anzhen Hospital from January to June 2017were included in this study, among which concluded 1 566 T2DM patients. The correlation between NLR and coronary artery disease in the population was analyzed by inter-group comparison of NLR values, the correlation between NLR and baseline indicators between the two groups, and the logistic regression analysis.Meanwhile, the linear relationship between NLR and coronary artery disease with T2DM was explored by restricted cubic spline. Results: NLR was significantly higher in patients with coronary artery disease and coronary artery disease complicated with T2DM than in the control group, and NLR was significantly positively correlated with traditional risk factors including gender, hypertension, age, smoking, fasting blood glucose and creatinine in both groups. Univariate and multivariate logistic regression analysis indicated that NLR was significantly correlated with coronary artery disease in both groups(multivariate regression model 3:overall population: OR=1.089,95%CI:1.014-1.170,P=0.019;T2DM population: OR=1.121,95%CI:1.003-1.253,P=0.045). Conclusions: As a clinically readily available biomarker, NLR is independently associated with elderly patients with coronary artery disease complicated with T2DM.
