摘要
目的调查新疆地区母源CD36抗原缺失情况,并分析母源CD36抗原缺失与新生儿发生同种免疫血小板减少症(NAIT)的关系。方法选取2019年5月至2020年6月我院产科门诊就诊的610例孕妇作为研究对象,于就诊当天检测孕妇CD36抗原、血小板抗体,统计新疆地区主要民族[维吾尔族、汉族]孕妇CD36抗原缺失情况及血小板抗体阳性率;于新生儿出生24h内检测其血小板计数(PLT),统计NAIT发生情况。结果纳入的610例孕妇,经检测CD36抗原阳性占98.69%,CD36抗原缺失占1.31%,均为Ⅱ型抗原缺失,无Ⅰ型抗原缺失;汉族、维吾尔族孕妇CD36抗原缺失情况比较,差异无统计学意义(P>0.05)。汉族、维吾尔族孕妇血小板抗体阳性率比较,差异无统计学意义(P>0.05)。汉族新生儿出生24 h内PLT高于维吾尔族,差异有统计学意义(P<0.05);两族新生儿其他资料比较,差异无统计学意义(P>0.05)。结论新疆地区孕妇存在Ⅱ型CD36抗原缺失,且汉族、维吾尔族孕妇CD36抗原缺失率无显著性差异,血小板抗体、CD36抗体阳性率无显著性差异。
Objective To investigate the deletion of maternal CD36 antigen in Xinjiang, and to analyze the relationship between maternal CD36 antigen deletion and neonatal alloimmunothrombocytopenia(NAIT). Methods 610 pregnant women admitted to our hospital’s obstetric outpatient clinic from May 2019 to June 2020 were selected as the research objects,CD36 antigen and platelet antibody of pregnant women were detected on the day of admission. The deletion of CD36 antigen and the positive rate of platelet antibody in pregnant women of major nationalities [Uygur and Han] in Xinjiang were counted.The platelet count(PLT) was measured of newborns within 24 hours after birth, and the incidence of NAIT was counted. Results Among the 610 included pregnant women, the positive rate of CD36 antigen was 98.69%, and the missing rate of CD36antigen was 1.31%, all of them were type II antigen deletion, and there was no type I antigen deletion;compared CD36 antigen deletion between Han and Uygur pregnant women, there was no statistical significant difference(P>0.05). Compared the positive rate of platelet antibody between Han and Uygur pregnant women, there was no statistical significant difference(P>0.05). PLT of newborns of Han nationality was higher than that of Uygur nationality within 24 hours, the difference was statistically significant(P<0.05). Compared other data of newborns in both groups, there was no statistical significant difference(P>0.05). Conclusion There is a deletion of type II CD36 antigen in pregnant women in Xinjiang, and there is no significant difference in CD36 antigen deletion rate, platelet antibody and CD36 antibody positive rate between Han and Uygur pregnant women.
