下段输尿管结石输尿管镜碎石术中旁置F3和F5输尿管导管效果比较

A comparative analysis of the effects of F3 and F5 catheter placed beside the ureteroscope during ureteroscopic lithotripsy for lower ureteral stones

目的分析行输尿管镜碎石术中输尿管镜旁留置F3和F5输尿管导管对下段输尿管结石患者疗效的影响。方法回顾性分析2019年1月-2021年1月在山东第一医科大学第二附属医院泌尿外科行输尿管镜碎石术并于术中旁置F3或F5输尿管导管的94例下段输尿管结石患者的临床资料,根据旁置导管不同分为F3导管组及F5导管组,比较两组相关数据,并进行统计学分析,术后定期随访。结果两组患者术前一般资料及结石相关资料对比差异均无统计学意义。F5导管组较F3导管组手术时间[(21.15±4.94)min vs.(31.06±8.47)min]、术后发热发生率(6.5%vs.20.8%)、术后降钙素原水平[(0.38±0.12)ng/mL vs.(0.64±0.37)ng/mL]、术后第1天肉眼血尿发生率(39.1%vs.64.6%)、术中结石逃逸率(2.2%vs.14.6%)、残石率(2.2%vs.16.7%)等指标更优。F5导管组无术后输尿管狭窄病例,F3导管组出现1例术后输尿管狭窄病例。结论在下段输尿管结石输尿管镜碎石术中旁置F5输尿管导管相比F3输尿管导管可更好地提高手术疗效及安全性。

Objective To compare the effects of F3 and F5 catheter placed beside the ureteroscope in ureteroscopic lithotripsy for lower ureteral stones.Methods Clinical data of 94 patients with lower ureteral calculi treated in our hospital during Jan.2019 and Jan.2021 with F3 or F5 catheter placed beside the ureteroscope in ureteroscopic lithotripsy were retrospectively studied.The patients were divided into F3 group and F5 group,and the relevant data were compared and statistically analyzed.Results There were no significant differences in the baseline data between the two groups.The F5 group had shorter operation time[(21.15±4.94)min vs.(31.06±8.47)min],lower incidence of postoperative fever(6.5%vs.20.8%),lower level of procalcitonin level[(0.38±0.12)ng/mL vs.(0.64±0.37)ng/mL],lower incidence of gross hematuria on postoperative day 1(39.1%vs.64.6%),lower intraoperative stone escape rate(2.2%vs.14.6%),and lower residual stone rate(2.17%vs.16.67%)than the F3 group.There was no postoperative ureteral stricture in the F5 group but 1 case in the F3 group.Conclusion Compared with F3 ureteral catheter,F5 catheter can better improve the efficacy and safety of ureteroscopic lithotripsy for lower ureteral stones.
