磁共振成像检查中磁敏感加权成像及弥散加权成像序列用于新生儿缺氧缺血性脑病的诊断 被引量:2

Analysis of the diagnostic efficacy of SWI and DWI sequence in MRI examination for neonatal hypoxic-ischemic encephalopathy

摘要 目的探究磁共振成像(MRI)检查中磁敏感加权成像(SWI)及弥散加权成像(DWI)序列用于新生儿缺氧缺血性脑病的诊断效能。方法选取2018年1月至2021年7月我院收治的90例新生儿缺氧缺血性脑病患儿,所有患儿均行常规MRI、SWI及DWI序列检查。以患儿临床症状表现为标准,分析MRI诊断新生儿缺氧缺血性脑病的临床价值;分别比较常规序列与SWI及DWI序列对新生儿缺氧缺血性脑病的诊断价值。结果MRI对新生儿缺氧缺血性脑病诊断的灵敏度为52.4%(33/63),特异度为18.5%(5/27),诊断符合率为42.2%(38/90)。常规序列对新生儿脑损伤病灶检出个数为82个,DWI检出163个病灶,DWI序列明显优于常规序列(P<0.05)。常规序列对新生儿颅内出血检出病灶为169个,SWI序列检出73个,常规序列明显优于SWI序列(P<0.05)。结论DWI序列对新生儿脑损伤病灶的诊断具有较好的诊断率,SWI序列对新生儿缺氧缺血性脑病脑室出血具有较高敏感性,临床工作中,将上述检查方式联合使用,可以对早期新生儿缺氧缺血性脑病诊断及对患儿预后进行评估。 Objective To explore the diagnostic efficacy of susceptibility weighted imaging(SWI)and diffusion weighted imaging(DWI)sequences in MRI examinations for neonatal hypoxic-ischemic encephalopathy.Methods A total of 90 neonates with hypoxic-ischemic encephalopathy in our hospital from January 2018 to July 2021 were selected.All the children underwent routine MRI,SWI and DWI serial examinations.Based on the clinical symptoms of children,the clinical value of MRI in diagnosing neonatal hypoxic-ischemic encephalopathy was analyzed;the diagnostic value of conventional sequence and SWI and DWI sequence for neonatal hypoxic-ischemic encephalopathy were compared.Results The sensitivity of MRI for the diagnosis of neonatal hypoxic-ischemic encephalopathy was 52.4%(33/63),the specificity was 18.5%(5/27),and the diagnostic coincidence rate was 42.2%(38/90).The number of neonatal brain injury lesions detected by the conventional sequence was 82,and 163 lesions were detected by DWI.The DWI sequence was significantly better than the conventional sequence(P<0.05).The conventional sequence detected 169 lesions in neonatal intracranial hemorrhage,and the SWI sequence detected 73.The conventional sequence was significantly better than the SWI sequence(P<0.05).Conclusion The DWI sequence has a good diagnostic rate for the diagnosis of neonatal brain injury lesions,and the SWI sequence has a high sensitivity to neonatal hypoxic-ischemic encephalopathy ventricular hemorrhage.In clinical work,the combination of the above examination methods can be used for early Diagnosis of neonatal hypoxic-ischemic encephalopathy and evaluation of the prognosis of children.
