摘要 目的:了解居民慢性萎缩性胃炎现状,探讨慢性萎缩性胃炎影响因素,为慢性萎缩性胃炎的防控提供科学依据。方法:采用多阶段整群抽样方法,随机选取2020年5月1日~2021年8月31日完成胃镜检查的486例患者为研究对象;使用一般资料调查表、睡眠状况自评量表(SRSS)、焦虑自评量表(SAS)、抑郁自评量表(SDS)、食物频率法问卷(FFQ)收集研究对象的基本资料和过去1年的食物摄入情况;采用t检验、χ^(2)检验和Logistics多因素回归分析慢性萎缩性胃炎的影响因素及关系。结果:研究对象的受教育程度、胃病家族史、幽门螺旋杆菌、睡眠情况、焦虑情况和抑郁情况是慢性萎缩性胃炎的影响因素(P<0.05);粗粮、咸菜类、油炸食品、酒类、盐与慢性萎缩性胃炎呈正相关(P<0.05),是慢性萎缩性胃炎发生的危险性因素;叶类蔬菜、非叶类蔬菜、新鲜水果、蛋类、奶及其制品与慢性萎缩性胃炎呈负相关(P<0.05),是慢性萎缩性胃炎发生的保护性因素。结论:慢性萎缩性胃炎的影响因素是多样的,良好健康平衡的饮食和生活习惯可有效预防慢性萎缩性胃炎的发生。 Objective:To explore the current status and influencing factors of chronic atrophic gastritis in residents,so as to provide scientific basis for the prevention and control of chronic atrophic gastritis.Methods:With multi-stage cluster sampling method,486 patients who completed gastroscopy during May 1,2020 to August 31,2021 were randomly selected as the research objects.The basic information and food intake in the past year were collected by general information questionnaire,sleep status self-assessment scale(SRSS),anxiety self-assessment scale(SAS),depression self-assessment scale(SDS)and food frequency questionnaire(FFQ).The influencing factors and relationships of chronic atrophic gastritis were analyzed by t test,χ^(2) test and multivariate Logistics regression.Results:The educational level,family history of gastropathy,Helicobacter pylori,sleep,anxiety and depression were the influencing factors for chronic atrophic gastritis(P<0.05).Coarse grain,pickles,fried food,alcohol and salt were significantly positively correlated with chronic atrophic gastritis(P<0.05),which were risk factors for the chronic atrophic gastritis.Leafy vegetables,non-leaf vegetables,fresh fruits,eggs,milk and their products wer ignificantly negatively correlated with chronic atrophic gastritis(P<0.05),which were protective factors for the occurrence of chronic atrophic gastritis.Conclusion:The influencing factors of chronic atrophic gastritis are diverse,and a good,healthy and balanced diet and living habits can effectively prevent the occurrence of chronic atrophic gastritis.
