通脉降浊方加减对冠心病稳定性心绞痛患者的疗效分析

Curative Effect of Modified Tongmai Jiangzhuo Prescription in Patients with Coronary Heart Disease and Stable Angina Pectoris

目的:探究对冠心病稳定型心绞痛患者施行通脉降浊方加减治疗的效果。方法:选用随机数字表法将2019年5月-2020年10月仙桃市中医医院106例冠心病稳定型心绞痛患者分为对照组(西药治疗,n=53)、观察组(西药+通脉降浊方加减治疗,n=53)。比较两组总有效率、血脂指标、中医症状(气短痰多、胸痛、胸闷、心悸)积分、血流动力学指标、血清白细胞介素-6(IL-6)、单核细胞趋化蛋白-1(MCP-1)水平。结果:(1)观察组总有效率高于对照组(P<0.05);(2)观察组治疗后胸痛、胸闷、气短痰多及心悸评分均较对照组低(P<0.05);(3)观察组治疗后TC、TG、LDL-C水平均较对照组低,且HDL-C水平较对照组高(P<0.05);(4)观察组治疗后纤维蛋白原、全血低切黏度、全血高切黏度及血浆黏度均较对照组低(P<0.05);(5)观察组治疗后IL-6、MCP-1水平均较对照组低(P<0.05)。结论:对冠心病稳定型心绞痛患者施行通脉降浊方加减治疗有助于增强治疗效果,降低血脂水平,改善胸闷、胸痛等症状,并有助于调节血流动力学,减轻炎症反应,可行性较高。

Objective:To explore the effect of Tongmai Jiangzhuo Prescription in patients with stable angina pectoris of coronary heart disease.Method:From May 2019 to October 2020,106 patients with stable angina pectoris of coronary heart disease in Xiantao Hospital of Traditional Chinese Medicine were divided into control group(treated with western medicine,n=53)and observation group(treated with western medicine+Tongmai Jiangzhuo Prescription,n=53)by random number table method.The total effective rate,blood lipid indexes,scores of traditional Chinese medicine(TCM)symptoms(shortness of breath phlegm,chest pain,chest tightness,palpitations),hemodynamic indexes,serum interleukin-6(IL-6)and monocyte chemotaxis protein-1(MCP-1)levels were compared between the two groups.Result:(1)The total effective rate of the observation group was higher than that of the control group(P<0.05).(2)After treatment,the scores of chest pain,chest tightness,shortness of breath phlegm and palpitations in the observation group were lower than those in the control group(P<0.05).(3)After treatment,the levels of TC,TG and LDL-C in the observation group were lower than those in the control group,and HDL-C was higher than that in the control group(P<0.05).(4)Fibrinogen,whole blood low shear viscosity,whole blood high shear viscosity and plasma viscosity in the observation group were lower than those in the control group after treatment(P<0.05).(5)The levels of IL-6 and MCP-1 in the observation group were lower than those in the control group after treatment(P<0.05).Conclusion:It is feasible to apply Tongmai Jiangzhuo Prescription to patients with stable angina pectoris of coronary heart disease,which can enhance the therapeutic effect,reduce the level of blood lipid,improve the symptoms of chest tightness and chest pain,and help to regulate hemodynamics and reduce inflammation.
