肺癌放射性肺炎严重程度的影响因素分析

Influencing factors of severity of radiation pneumonia in patients with lung cancer

目的探讨不同因素对肺癌放射性肺炎严重程度的影响。方法回顾性研究数据来自于2010年1月至2021年10月因肺癌放疗,发生放射性肺炎入住山东大学齐鲁医院的116例肺癌患者,收集患者的临床资料。使用SPSS 24.0软件进行数据处理和统计分析,多因素Logistic回归分析用于评估相关因素对放射性肺炎严重程度的影响。结果入组的116例患者中,男性89(76.7%)人,女性27(23.3%)人;平均年龄62.73±9.99岁(范围:39~83)。单因素分析结果显示:性别(P=0.009)、吸烟指数(P<0.001)和有无饮酒史(P=0.003)与放射性肺炎严重程度显著相关。多因素分析后,显示仅有吸烟指数与放射性肺炎严重程度相关。结论吸烟指数≥400的肺癌患者发生放射性肺炎时,严重程度分级越高,吸烟指数可以作为放射性肺炎不良预后的预测指标。

Objective To explore the influence of different factors on severity grading of lung cancer radiation pneumonia.Methods The retrospective study data were collected from 116 lung cancer patients admitted to Qilu Hospital of Shandong University from January 2010 to October 2021 who developed radiation pneumonia due to lung cancer radiotherapy.Their clinical data were collected.Results Among 116 enrolled patients,89(76.7%)were male and 27(23.3%)were female.The mean age was 62.73±9.99 years old,ranging from 39 to 83.The smoking index was less than 400 in 34 patients(29.3%).87 had a smoking index of 400 or higher(70.7%).Univariate analysis showed that gender(P=0.009),smoking index(P<0.001)and history of alcohol consumption(P=0.003)were significantly correlated with the severity of radiation pneumonia.After multivariate analysis,only smoking index was associated with the severity of radiation pneumonia.Conclusion The severity of radiation pneumonia in lung cancer patients with smoking index≥400 is higher and smoking index can be used as a predictor of poor prognosis of radiation pneumonia.
