血尿酸、B型脑钠肽及中性粒细胞/淋巴细胞比值水平在慢性阻塞性肺疾病并发肺动脉高压中的表达及其意义

Expression and significance of levels of serum uric acid,BNP and NLR in patients with chronic obstructive pulmonary disease complicated with pulmonary hypertension

目的探讨血尿酸(UA)、B型脑钠肽(BNP)及中性粒细胞/淋巴细胞比值(NLR)水平在慢性阻塞性肺疾病(COPD)并发肺动脉高压(PH)中的表达及其意义。方法我院治疗的COPD并发PH患者114例,根据PH程度不同分为轻度组31例、中度组48例、重度组35例。选取同期治疗的单纯COPD者101例(单纯COPD组)。对比不同人群UA、BNP及NLR水平,分析UA、BNP及NLR与肺动脉高压分级相关性。结果COPD并发PH组UA、BNP及NLR水平均高于COPD组(P<0.05);重度组UA、BNP及NLR水平高于轻度与中度组(P<0.05);UA、BNP及NLR与肺动脉压呈正相关(P<0.05)。结论血UA、BNP及NLR在COPD并发PH中表达均明显升高,与患者肺动脉高压程度为正相关,可为COPD并发PH患者病情评估提供参考。

Objective To investigate the expression and significance of serum uric acid(UA),B-type brain natriuretic peptide(BNP)and neutrophil/lymphocyte ratio(NLR)in chronic obstructive pulmonary disease(COPD)complicated with pulmonary hypertension(PH).Methods One hundred and fourteen patients with COPD complicated with PH treated in our hospital from February 2017 to June 2021 were selected.The patients were divided into a mild group(n=31),a moderate group(n=48)and a severe group(n=35)according to PH degree.In addition,101 patients with simple COPD treated in our hospital during the same period were selected as a simple COPD group.The levels of UA,BNP and NLR in different populations were compared,and the correlation between the levels of UA,BNP and NLR and the pulmonary hypertension grading was analyzed.Results The levels of UA,BNP and NLR in the COPD combined with PH group were significantly higher than those in the simple COPD group(P<0.05).The levels of UA,BNP and NLR in the severe group were significantly higher than those in the mild and the moderate groups(P<0.05).UA,BNP and NLR were positively correlated with pulmonary arterial pressure(P<0.05).Conclusion The expressions of blood UA,BNP and NLR in patients with COPD complicated with pH are significantly increased,which is positively correlated with pulmonary hypertension grading.These indexes can provide a reference for the evaluation of COPD complicated with PH.
