集束化康复护理对老年股骨颈骨折患者术后心理和日常生活能力的影响

The effect of cluster rehabilitation nursing on postoperative psychological status and daily living ability in elderly patients with femoral neck fracture

目的探讨集束化康复护理作为老年股骨颈骨折患者术后标准护理措施的有效性。方法选择2019年1月至2020年2月于四川省骨科医院行手术治疗的老年股骨颈骨折患者88例,采用区组化随机方式分为对照组和试验组各44例。对照组给予常规护理,试验组给予集束化康复护理。比较两组患者干预后的精神、心理相关指标,髋关节功能评分,日常生活能力,满意度及并发症发生率。结果护理干预后两组患者焦虑自评量表(SAS)和抑郁自评量表(SDS)评分均显著改善,且试验组优于对照组组(P<0.05);试验组患者功能独立性评定量表(FIM)评分、Harris评分、日常生活能力量表评分(Barthel指数)、患者满意度均显著优于对照组(P<0.05);且试验组SCL-90评分中抑郁、焦虑、偏执三个维度的分值显著优于对照组(P<0.05)。结论经集束化康复护理干预,可有效改善老年股骨颈骨折患者精神、心理状态,有利于患者髋关节功能和日常生活能力恢复。

Objective To explore the effectiveness of cluster rehabilitation nursing as a standard postoperative nursing measure for elderly patients with femoral neck fracture.Methods A total of 88 elderly patients with femoral neck fractures admitted to our hospital from January 2019 to February 2020 were randomly divided into a control group and a study group,44 in each group.The mental and psychological status,hip function score,activities of daily living,satisfaction and incidence of complications after intervention were compared between the two groups.Results The scores of self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were significantly improved in both groups after intervention,and the study group was superior to the control group.Functional independence measure(FIM)score,Harris score,Barthel index,and patients’satisfaction in the study group were significantly better than those in the control group(P<0.05).The scores of the three dimensions of depression,anxiety,and paranoid ideation in the SCL-90 score of the study group were significantly better than those of the control group(P<0.05).Conclusion The cluster rehabilitation nursing intervention can effectively improve the mental and psychological status of elderly patients with femoral neck fracture.It is beneficial to the recovery of hip joint function and living ability.
