癫痫患者血浆miR-542-3p TLR4水平及临床意义 被引量:2

Plasma miR-542-3p and TLR4 levels in patients with epilepsy and their clinical significance

摘要 目的探讨癫痫患者血浆miR-542-3p、Toll样受体4水平及其对癫痫发作类型的预测价值。方法对134例原发性癫痫患者(观察组)和140名健康志愿者(对照组)应用实时荧光定量聚合酶链式反应测定血浆miR-542-3p水平,应用酶联免疫吸附实验测定血浆Toll样受体4水平。应用受试者工作特征曲线分析癫痫患者血浆miR-542-3p、Toll样受体4对癫痫不同发作类型的预测价值,应用Pearson相关分析探讨miR-542-3p与Toll样受体4的相关性。结果观察组血浆miR-542-3p水平显著低于对照组(P<0.01),Toll样受体4水平显著高于对照组(P<0.01)。全面性发作患者血浆miR-542-3p水平显著低于部分性发作患者(P<0.01),Toll样受体4水平显著高于部分性发作患者(P<0.01)。受试者工作特征曲线显示,血浆miR-542-3p预测癫痫不同发作类型的曲线下面积为0.886,截断值为0.838,敏感度为87.3%,特异度为76.2%;Toll样受体4预测癫痫不同发作类型的曲线下面积为0.803,截断值为2.285 ng·ml^(-1),敏感度为83.1%,特异度为71.4%;两者联合检测预测癫痫不同发作类型的曲线下面积为0.926,敏感度为93.0%,特异度为85.7%。癫痫患者血浆miR-542-3p与Toll样受体4呈显著负相关(r=-0.610,P<0.05)。结论癫痫患者血浆miR-542-3p水平下降,Toll样受体4水平升高,两者呈显著负相关,对癫痫发作类型有一定的预测价值,未来或可作为预测癫痫发作类型的有效指标。 Objective To investigate the expression levels of microRNA-542-3p(miR-542-3p)and toll-like receptor 4(TLR4)in epilepsy patients and their predictive value for seizure types.Methods A total of 134 patients with primary epilepsy(observation group)and 140 healthy volunteers(control group)were selected as the research objects.The level of plasma miR-542-3p was measured by real-time quantitative reverse transcription PCR(qRT-PCR),and the level of plasma TLR4 was measured by enzyme-linked immunosorbent assay(ELISA).The predictive value of plasma miR-542-3p and TLR4 for different seizure types in patients with epilepsy was analyzed by receiver operating characteristic(ROC).Pearson correlation analysis was used to explore the correlation between miR-542-3p and TLR4.Results The level of plasma miR-542-3p in the observation group was significantly lower than that in the control group(P<0.01),and the level of TLR4 was significantly higher than that in the control group(P<0.01).The level of plasma miR-542-3p in patients with generalized seizures was significantly lower than that in patients with partial seizures(P<0.01),and the level of TLR4 was significantly higher than that in patients with partial seizures(P<0.01).ROC showed that the area under the curve of plasma miR-542-3p in predicting different seizure types was 0.886,the cut-off value was 0.838,the sensitivity was 87.3%,and the specificity was 76.2%.The area under the curve of TLR4 in predicting different seizure types was 0.803,the cut-off value was 2.285 ng·ml^(-1),the sensitivity was 83.1%,and the specificity was 71.4%.The area under the curve of the combined detection of the two predicting different seizure types was 0.926,the sensitivity was 93.0%,and the specificity was 85.7%.Plasma miR-542-3p in epilepsy patients was significantly negatively correlated with TLR4(r=-0.610,P<0.05).Conclusions The expression level of miR-542-3p in the plasma of epileptic patients is decreased,while TLR4 is increased.They are significantly negatively correlated,and they may be effective indexes to predict the type of epileptic seizure in the future.
