急性期脑卒中患者疾病进展恐惧影响因素分析

Analysis of risk factor of the fear of progression in acute stroke patients

目的分析脑卒中患者急性期疾病进展期恐惧影响因素。方法收集2020年10月~2021年3月河北省人民医院收治的脑卒中患者335例,对其采用一般资料调查表、疾病资料调查表、恐惧疾病进展简化量表、社会支持评定量表、家庭亲密度与适应性量表进行问卷调查。根据恐惧疾病进展简化量表得分,将患者分为心理功能失调组(≥34分)和非心理功能失调组(<34分)。结果335例急性期脑卒中患者疾病进展恐惧心理功能失调发生率42.7%(143/335)。经济状况为收入>支出、男性、经济状况为收支平衡、社会支持高、年龄大、实际家庭亲密度高是急性期脑卒中患者疾病进展恐惧的保护性因素(P<0.05);神经功能缺损程度高、自理能力(基本自理)、职业状态为在职、多发病灶、体质指数高、不满意家庭亲密度高是其危险因素(P<0.05)。结论经济状况、性别、社会支持、年龄和实际家庭亲密度是急性期脑卒中患者疾病进展恐惧的保护因素,神经功能缺损程度、自理能力、职业状态、病灶数量、体质指数和不满意家庭亲密度是其危险因素,医护人员可参照本研究结果制定干预措施,促进脑卒中患者心理健康。

Objective Analyze influence factors of the fear of progression of acute stage stroke patients.Methods From October 2020 to March 2021,335 stroke patients in acute stage,who from Hebei General Hospital,were selected as the research subjects.The General Information Questionnaire,Disease Information Questionnaire,Fear of Progression Questionnaire-Short Form,Social Support Rating Scale,Family Adaptability and Cohesion Scale were used to investigate.According to the score of FoP-Q-SF divided the research subjects into two groups,FoP-Q-SF≥34 were mental function disorder,on the contrary,FoP-Q-SF<34 were mental function not disorder.Results The incidence rate of mental function disorder of fear of progression in this research was 42.7%(143/335).Multivariate logistics regression analysis indicated that income>expenditure,male,income=expenditure,high social support level,elder,high actual family cohesion score were the protect factors of fear of progression of stroke patients(P<0.05),high NIH Stroke Scale score,60≤self-care ability score<100,employed,Multiple lesions,high body mass index,high unsatisfied with family cohesion score were the risk factors of fear of progression of stroke patients(P<0.05).Conclusion Financial sta-tus,gender,social support,age and actual family cohesion were the protect factors of fear of progres-sion with acute stage stroke patients,and NIH Stroke Scale score,self-care ability,employment sta-tus,number of lesions,body mass index,unsatisfied with family cohesion were the risk factors,med-ical staffs can consider this essay as reference to draw up intervention measures to promote stroke pa-tients’mental health.
