抗-Ec、抗-P_(1)合并抗-Jk^(a)致疑难配血分析

Analysis of Incompatibility in Blood Crossmatching Induced by Anti-Ec,Anti-P_(1) and Anti-Jk^(a)

目的探讨交叉配血中抗-Ec、抗-P_(1)合并抗-Jk^(a)抗体引起配血不合的原因。方法通过ABO血型鉴定、Rh分型、直接抗人球蛋白试验、不规则抗体筛查、抗体特异性鉴定及抗体剂量效应对患者配血不合进行分析。结果对照细胞谱格局表采用剂量效应打分制,该患者血清中检出抗-Ec、抗-P_(1)合并抗-Jk^(a)。结论患者血清中不规则抗体是导致交叉配血不合的主要原因,联合抗体应充分运用剂量效应以减少易漏检抗体机率,保证输血安全性。

Objective To investigate the reason of anti-Ec,anti-P_(1) combined with anti-Jk^(a) antibody inducing incompatibility in blood crossmatching.Methods By methods of ABO blood typing,Rh blood typing,direct Coombs testing,irregular antibody screening,antibody specificity identifying and antibody dose-effect were used to analyze the reason of incompatibility in blood crossmatching.Results The dose-effect grading system was used to compare the pattern of cell spectra,and anti-Ec,anti-P1 combined with anti-Jk^(a) antibodies in the serum of patient.Conclusion The presence of irregular antibodies in the serum of patients is the main factor,leading to crossmatch discord.Applying the dose-effect to the combined antibodies could reduce the probability of missing weak antibodies and ensure the safety of clinical transfusion.
