经脐单孔腹腔镜与传统腹腔镜手术治疗妊娠合并卵巢囊肿效果对比研究

Comparative Analysis of the Efficacy and Safety of Transumbilical Laparoendoscopic Single Site Surgery and Traditional Laparoscopic Surgery in the Treatment of Pregnant Patients with Ovarian Cyst

目的:探讨经脐单孔腹腔镜(TU-LESS)手术治疗妊娠合并卵巢囊肿的安全性和可行性。方法:回顾性分析2017年10月至2021年4月广州医科大学附属第三医院因妊娠合并卵巢囊肿行手术治疗患者共30例,根据手术方式分为观察组(TU-LESS手术组)16例和对照组(传统腹腔镜手术组)14例,比较两组的术中、术后情况以及妊娠结局。结果:观察组住院费用更少,差异有统计学意义(P<0.05);两组手术类型、手术时间、术中出血量、住院总天数、术后住院天数差异无统计学意义(P>0.05);两组术后感染指标、术后止痛药使用频率、术后肛门排气时间、术后早期并发症发生率差异无统计学意义(P>0.05);两组妊娠结局、分娩方式以及分娩孕周差异无统计学意义(P>0.05)。两组均无新生儿窒息发生。结论:TU-LESS手术治疗妊娠合并卵巢囊肿具有与传统腹腔镜手术相似的手术效果和妊娠结局,并且具有创伤更小、切口更美观、切除囊肿高效、标本取出方式更优、治疗费用更少的优势。

Objective:To analyze the efficacy and safety between transumbilical laparoendoscopic single site surgery and traditional laparoscopic surgery in the treatment of pregnant patients with ovarian cyst.Methods:Between October 2017 to April 2021,30 cases of pregnancy complicated with ovarian cyst were included in this research,who were randomly divided into the study group(n=16)and the control group(n=14).Patients in control group underwent transumbilical laparoendoscopic single site surgery,and patients in study group underwent traditional laparoscopic surgery.The therapeutic effects between this two groups was analysed and measured.Results:Patients in study group had less hospitalization costs than those in control group,the differences were statistically significant(P<0.05).When comparing operating time,blood loss during surgery,and average hospitalization time in the two groups,the result were not statistically significant(P>0.05).Also,no statistically significant differences were found between the two groups in postoperative infections,frequency of analgesic,anal exhaust time,and postoperative complication rate(P>0.05).The gestational age,neonatal outcomes and mode of delivery were not statistically significantly different between the two groups(P>0.05).Conclusions:The obvious advantages of single port laparoscopic surgery are less hospitalization costs,less postoperative pain,lower risks of incision-related complications and better cosmesis.In conclusion,transumbilical laparoendoscopic single site surgery can achieve similar clinical effects as traditional laparoscopy surgery.
