术前肌力训练对股骨头坏死患者全髋关节置换术后的影响

Effect of preoperative muscle strength training on patients with femoral head necrosis after total hip arthroplasty

目的探讨术前肌力训练对股骨头坏死患者全髋关节置换术后髋关节功能、生存质量及并发症的影响。方法前瞻性选取2018年1月至2021年4月靖江市人民医院收治的40例股骨头坏死拟行全髋关节置换术的患者,按照随机数字表法将其分为观察组和对照组,每组各20例。对照组患者采取术前常规护理,观察组患者在此基础上术前4周采取肌力训练。两组患者均进行全髋关节置换术。统计两组患者干预前及手术后的Harris髋关节功能评分(Harris hip score,HHS)、健康调查量表36(36-item short form health survey,SF-36)8个项目的评分及术后并发症发生情况。结果手术后,两组患者在HHS与SF-36的8个项目评分均高于本组干预前(P<0.05);观察组患者以上指标各项评分均显著高于对照组(P<0.05)。观察组患者的并发症发生率显著低于对照组(P<0.05)。结论术前肌力训练能改善股骨头坏死患者全髋关节置换术后的髋关节功能,提高患者的生存质量,降低术后并发症发生率。

Objective To explore the effect of preoperative muscle strength training on hip function,quality of life and complications after total hip arthroplasty for femoral head necrosis patients.Methods Forty patients undergoing total hip arthroplasty for femoral head necrosis in Jingjiang People's Hospital from January 2018 to April 2021 were prospectively selected divided into observation group(n=20)and control group(n=20)according to the random num-ber table method.The patients of control group received routine preoperative nursing,and the patients of observation group received muscle strength training for a total of 4 weeks before surgery.Harris hip score(HHS),eight items in the 36-item short form health survey(SF-36)and postoperative complications of patients in two groups were ana-lyzed before intervention and after surgery.Results After surgery,the scores of HHS and eight items in the SF-36 in two groups were improved compared with those before intervention,with statistically significant differences(P<0.05).The scores of the observation group were higher than those of the control group(P<0.05).The incidence of complica-tions in the observation group was lower than that in the control group(P<0.05).Conclusions Preoperative muscle strength training can improve the hip function and the quality of life,and reduce the incidence of postoperative com-plications in patients with femoral head necrosis after total hip arthroplasty.
