槐杞黄颗粒对急性淋巴细胞白血病维持化疗患儿免疫功能影响的临床研究

A clinical study on the effect of Huaiqihuang granules on immune function during maintenance therapy in children with acute lymphoblastic leukemia

目的探讨槐杞黄颗粒对儿童急性淋巴细胞白血病(acute lymphoblastic leukemia,ALL)患儿维持治疗期间免疫功能影响。方法选择2019年1月至2021年1月大连市儿童医院确诊为ALL开始进入维持化疗患儿40例为研究对象,采用数字法随机分为常规化疗组(对照组)及常规化疗+槐杞黄颗粒组(观察组),每组各20例。对照组按照化疗方案按期给予“6-MP+MTX+VDS+DX”治疗,观察组在常规化疗基础上分别予槐杞黄颗粒辅助治疗6个月,定期随访6个月。观察两组的感染发生率,监测免疫功能(免疫球蛋白及CD4^(+),CD8^(+))的变化。结果两组化疗前体液免疫功能各项指标检测比较,差异无统计意义(P>0.05);分别于开始维持化疗2周、1个月、3个月、6个月进行相关指标检测,维持化疗期间对照组感染率明显高于观察组;对照组及观察组细胞免疫功能及体液免疫比较,相同测定时间点对照组CD4+及IgG、IgM、IgA的值低于观察组。结论通过对急性淋巴细胞白血病患儿维持化疗期间不间断口服槐杞黄颗粒6个月,患儿感染率降低,体液及细胞免疫功能部分提高,提高了其长期无病生存率,改善生存质量。

Objective To investigate the effect of Huaiqihuang granules on immune function during maintenance therapy for children with acute lymphoblastic leukemia(ALL).Methods A total of 40 children with ALL diagnosed in Dalian Children's Hospital from January 2019 to January 2021 were selected as the research subjects.They were randomly divided into the control group(conventional chemotherapy group)and the observation group(conventional chemotherapy+Huaiqihuang granules group),with 20 cases in each group.The control group was given“6-MP+MTX+VDS+DX”according to the chemotherapy regimen,and the observation group was given Huaiqihuang granules adjuvant treatment for 6 months on the basis of conventional chemotherapy,and was followed up regularly for 6 months.The incidence of infection was observed,and the changes of immune function(immunoglobulin,CD4+and CD8+)were monitored.The effect of Huaiqihuang granules on the immune system of children with ALL maintenance period were compared.Results Cellular immunity and humoral immunity were performed in the control group and the observation group before routine maintenance chemotherapy,and the results showed no statistical significance(P>0.05).The cellular immunity and humoral immunity of the control group and the observation group were compared at the beginning of maintenance chemotherapy two weeks from January to March and June,respectively.The infection rate of control group was significantly higher than observation group during maintenance chemotherapy,the CD4+,IgG,IgM and IgA values of the control group were lower than those of the observation group at the same time point.Conclusion Through the continuous oral administration of Huaiqihuang granules for 6 months during the chemotherapy period,the infection rate of children with acute lymphocytic leukemia was reduced,the humoral and cellular immune function was improved partly,the long-term disease-free survival rate was improved,and the quality of life was improved.
