摘要
目的 探讨血清糖类抗原199(CA199)、糖类抗原125(CA125)、癌胚抗原(CEA)、肿瘤坏死因子-α(TNF-α)联合检测在炎症性肠病早期诊断中的应用价值。方法 选取78例炎症性肠病患者作为炎症性肠病组, 40例结直肠癌患者作为结直肠癌组;选择同期80例接受体检的健康者作为健康对照组。三组均开展血清CA199、CA125、CEA、TNF-α检测。比较三组血清CA199、CA125、CEA、TNF-α水平;以病理学结果作为金标准,分析血清CA199、CA125、CEA、TNF-α联合检测诊断炎症性肠病的准确度、灵敏度、特异度。结果 结直肠癌组血清CA199、CA125、CEA、TNF-α水平高于炎症性肠病组和健康对照组,炎症性肠病组高于健康对照组,三组比较差异均具有统计学意义(P<0.05)。血清CA199、CA125、CEA、TNF-α联合检测诊断炎症性肠病的准确度为89.87%(142/158),灵敏度为92.31%(72/78),特异度为87.50%(70/80)。结论 血清CA199、CA125、CEA、TNF-α联合检测在炎症性肠病早期诊断中的应用价值显著,有利于疾病得到早期诊治。
Objective To discuss the application value of combined detection of serum carbohydrate antigen 199(CA199),carbohydrate antigen 125(CA125),carcinoembryonic antigen(CEA),tumor necrosis factor-α(TNF-α)in early diagnosis of inflammatory bowel disease.Methods 78 patients with inflammatory bowel disease were selected as the inflammatory bowel disease group,and 40 patients with colorectal cancer were selected as colorectal cancer group;80 healthy subjects during the same period were selected as the healthy control group.Serum CA199,CA125,CEA and TNF-αwere detected in all three groups.The levels of serum CA199,CA125,CEA and TNF-αwere compared among the three groups;the accuracy,sensitivity and specificity of the combined detection of CA199,CA125,CEA and TNF-αfor inflammatory bowel disease were analyzed using pathological results as the gold standard.Results The levels of serum CA199,CA125,CEA,and TNF-αin the colorectal cancer group were higher than those in the inflammatory bowel disease group and the healthy control group,and the inflammatory bowel disease group was higher than the healthy control group.All the differences were statistically significant(P<0.05).The accuracy of the combined detection of serum CA199,CA125,CEA,and TNF-αfor inflammatory bowel disease was 89.87%(142/158),the sensitivity was 92.31%(72/78),and the specificity was 87.50%(70/80).Conclusion Combined detection of serum CA199,CA125,CEA,TNF-αhas significant application value in the early diagnosis of inflammatory bowel disease,which is conducive to the early diagnosis and treatment of the disease.
