SYSMEX尿干化学UC-3500与尿流式UF-4000在尿液检测中的应用评价 被引量:5

摘要 目的评价SYSMEX尿干化学UC-3500与尿流式UF-4000在尿液检测中的诊断效果。方法收集647例新鲜清洁中段尿,分别使用尿干化学UC-3500(简称尿干化学)及尿流式UF-4000(简称尿流式)进行检测,并采用人工镜检方式分别对两种仪器在灵敏度、特异性、假阳性率、假阴性率、符合率方面进行评价。结果在灵敏度上尿流式检测尿红细胞、白细胞、管型、细菌均优于尿干化学,分别为83.53%、93.13%、50.00%、88.54%;在特异性上尿干化学检测尿白细胞、亚硝酸盐优于尿流式,分别为71.25%、99.09%,但尿流式检测尿红细胞、管型上优于尿干化学,分别为90.99%、99.22%;在假阳性率上尿干化学检测尿红细胞及蛋白高于尿流式,分别为11.44%、24.73%;尿流式在检测尿白细胞及细菌上高于尿干化学,分别为20.29%、16.54%;假阴性率上检测尿蛋白时尿干化学及尿流式均较低,其他3项尿干化学检测高于尿流式,分别为10.82%、4.64%、8.19%;在符合率上尿干化学检测尿白细胞、亚硝酸盐上优于尿流式,分别为80.83%、91.04%,尿流式检测尿红细胞、管型检测上优于尿干化学,分别为89.03%、99.07%。结论SYSMEX尿干化学UC-3500及尿流式UF-4000在尿液检测中有着各自的优点及局限性,二者应联合分析,可以提高准确性并减少人工复检工作。 Objective To evaluate the diagnostic effect of SYSMEX urine dry chemistry UC-3500 and urine flow cytometry UF-4000 in urine detection.Methods 647 cases of fresh clean mid-stream urine samples were collected and tested by uc-3500(dry chemistry)and UF-4000(flow),respectively.The sensitivity,specificity,false positive rate,false negative rate and coincidence rate of the two instruments were evaluated by manual microscopy.Results The sensitivity of urine flow was better than that of urine dry chemistry in detecting items of red blood cells,white blood cells,tubular types and bacteria,which were 83.53%,93.13%,50.00%and 88.54%,respectively.For specificity,urine dry chemistry was better than urine flow chemistry in detecting items of urine white blood cell and nitrite(71.25%and 99.09%,respectively),while flow chemistry was better than urine dry chemistry in detecting item of urine red blood cell and tube type(90.99%and 99.22%,respectively).The false positive rate of urine red blood cell and protein detected by urine dry chemistry were 11.44%and 24.73%,respectively.Flow rate were 20.29%and 16.54%higher than dry chemistry.False negative rate were 10.82%,4.64%and 8.19%,respectively.In terms of coincidence rate,urine dry chemistry was superior to flow chemistry in urine white blood cell and nitrite detection(80.83%and 91.04%,respectively).Flow chemistry was superior to flow chemistry in urine red blood cell and tube type detection(89.03%and 99.07%,respectively).Conclusion SYSMEX urine dry chemistry UC-3500 and urine flow flow UF-4000 have different advantages and limitations in urine detection,and these systems should be utilized together to improve accuracy and reduce manual reexamination.
