自制血小板凝胶佐治糖尿病足临床观察

Clinical Observation of Self-Made Platelet Gel in the Adjuvant Treatment of Diabetic Foot

目的探讨自制血小板凝胶联合常规方法治疗糖尿病足的临床疗效。方法选取海南西部中心医院2018年8月至2021年8月收治的糖尿病足患者105例,按随机数字表法分为观察组(53例)和对照组(52例)。两组患者均予常规降糖、抗感染治疗,观察组患者加用自制血小板凝胶治疗,均连续治疗3个月。结果观察组总有效率为92.45%,显著高于对照组的76.92%(P<0.05)。治疗后,观察组患者的血管内皮生长因子,碱性成纤维细胞生长因子、胰岛素样生长因子-1水平均显著高于对照组(P<0.05);观察组患者的单核细胞趋化蛋白-1、Toll样受体4、高迁移率族蛋白1水平均显著低于对照组(P<0.05);观察组患者的糖尿病患者生存质量特异性量表(DMQLS)评分显著高于对照组(P<0.05)。观察组与对照组的不良反应发生率相当(9.43%比15.38%,P>0.05)。结论自制血小板凝胶联合常规方法治疗糖尿病足,可提升生长因子水平,降低炎性因子水平,提高患者的生活质量。

Objective To investigate the clinical efficacy of self-made platelet gel combined with conventional therapy in the treatment of diabetic foot.Methods A total of 105 patients with diabetic foot treated in the Hainan Western Central Hospital from August 2018 to August 2021 were selected and divided into the observation group(53 cases)and the control group(52 cases)by the random number table method.The patients in the two groups were treated with conventional hypoglycemic and anti-infective therapy,on this basis,the patients in the observation group were treated with self-made platelet gel.Both groups were continuously treated for three months.Results The total effective rate in the observation group was 92.45%,which was significantly higher than 76.92%in the control group(P<0.05).After treatment,the levels of vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF)and insulin-like growth factor-1(IGF-1)in the observation group were significantly higher than those in the control group(P<0.05),the levels of inflammatory monocyte chemoattractant protein-1(MCP-1),Toll-like receptor-4(TLR4)and high mobility group protein-1(HMG1)in the observation group were significantly lower than those in the control group(P<0.05),while the score of Diabetes Mellitus Quality of Life Scale(DMQLS)in the observation group was significantly higher than that in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(9.43%vs.15.38%,P>0.05).Conclusion The self-made platelet gel combined with conventional therapy in the treatment of diabetic foot can increase the levels of growth factors,reduce the levels of inflammatory factors,and improve the patients'quality of life.
