摘要
目的 探讨中老年(年龄51~89岁)外周动脉疾病(peripheral arterial disease, PAD)患者动态动脉硬化指数(ambulatory arterial stiffness index, AASI)与骨密度(bone mineral density, BMD)的关系,分析其发生骨质疏松(osteoporosis, OP)的危险因素。方法 选自2018年8月-2020年8月吉林省一汽总医院就诊的197例PAD患者(年龄51~89岁),均进行动态血压监测,以AASI>0.55为AASI增高组(140例),AASI≤0.55为AASI正常组(57例);测量患者腰椎BMD,并分为骨量减少组(58例)及骨质疏松组(139例);分析AASI与BMD的相关性、中老年PAD患者发生骨质疏松的危险因素。结果 AASI增高组年龄、血糖等均高于AASI正常组(P <0.05)。腰椎BMD低于AASI正常组(P <0.05);AASI与腰椎BMD呈负相关(r=-0.524,P <0.05);高龄、血糖升高、AASI增高是中老年PAD患者发生骨质疏松的独立危险因素(P <0.05),而非低体重、非钙缺乏是中老年PAD患者发生骨质疏松的保护因素(P <0.05)。结论 AASI与腰椎BMD呈正相关,AASI是中老年PAD患者发生骨质疏松的危险因素。
Objective To investigate the relationship between ambulatory arterial stiffness index(AASI) and bone mineral density(BMD) in middle-aged and elderly(aged 51~89 years) patients with peripheral arterial disease(PAD), and to analyze the incidence of bone mineral density. Risk factors for osteoporosis(OP). Methods From August 2018 to August 2020, 197 PAD patients(aged 51~89 years)in our hospital were monitored for ambulatory blood pressure, with AASI>0.55 as the increased AASI group(140 cases) and AASI≤0.55 as the normal AASI group(57 cases). Lumbar BMD was measured and divided into osteopenia group(58 cases) and osteoporosis group(139 cases). To analyze the correlation between AASI and BMD, and the risk factors of osteoporosis in middle-aged and elderly patients with PAD. Results The age and blood sugar in the increased AASI group were higher than those in the normal AASI group(P <0.05). The BMD of lumbar spine was lower than that of AASI normal group(P <0.05). AASI was negatively correlated with lumbar spine BMD(r =-0.524, P <0.05). Old age,elevated blood sugar and elevated AASI were independent risk factors for osteoporosis in middleaged and elderly PAD patients(P <0.05), while non-low body weight and non-calcium deficiency were protective factors for osteoporosis in middle-aged and elderly PAD patients(P <0.05). Conclusion There is a positive correlation between AASI and lumbar BMD, and AASI is a risk factor for osteoporosis in middle-aged and elderly patients with PAD.
