奥洛他定等药物联合治疗重度角膜缘型VKC的意义

The Significance of Olopatadine and Other Drugs Combined in the Treatment of Severe Limbal VKC

目的 探讨奥洛他定滴眼液等药物联合治疗重度角膜缘型春季角结膜炎(VKC)的临床疗效分析。方法 纳入我院于2019年8月-2020年8月收治的重度角膜缘型VKC患者67例(共134只眼),通过抛硬币法分为研究组和对照组。给予对照组埃美丁治疗,研究组采用奥洛他定联合0.1%氟米龙滴眼液和妥布霉素地塞米松眼膏进行治疗,对比两组患者治疗前后眼压、最佳矫正视力(BCVA)及临床疗效差异。结果 研究组患者经治疗14d、28d后,两眼眼压均较治疗前有所提升,较同时期对照组眼压明显高,治疗有效率和治愈率均显著高于对照组(t=17.6499,13.7223,14.3459,23.5240,P <0.05);两组患者治疗14d、28d后,两眼的BCVA对比治疗前视力均无统计学意义(P>0.05);研究组患者治疗14d、28d后,治疗有效率和治愈率均显著高于对照组(χ^(2)=9.9175,14.3261,17.1643,26.3243,P <0.05);研究组经28d治疗后,复发率5.88%明显低于对照组30.00%(χ^(2)=5.5138,P <0.05)。结论 针对重度角膜缘型患者采用奥洛他定联合0.1%氟米龙滴眼液和妥布霉素地塞米松眼膏进行治疗,可显著提升临床疗效,且治愈率较高,复发率较低,值得推广。

Objective To investigate the clinical effect of olopatadine eye drops and other drugs in the treatment of severe limbal vernal keratoconjunctivitis(VKC). Methods A total of 67 patients(134eyes) with severe limbal VKC who were admitted to our hospital from August 2019 to August 2020 were divided into the study group and the control group by coin toss method. The patients in the control group were treated with emetine, while those in the research group were treated with olopatadine combined with 0.1% fluoroMi Long eye drops and tobramycin dexamethasone eye ointment. The intraocular pressure, best corrected visual acuity(BCVA) and clinical efficacy were compared between the two groups before and after the treatment. Results After 14 or 28 days of treatment, the intraocular pressure in both eyes of patients in the study group was increased, which was significantly higher than that in the control group at the same period. The treatment efficiency and cure rate were significantly higher than those in the control group(t =17.6499, 13.7223,14.3459,23.5240, P <0.05). After 14 and 28 days of treatment, there was no statistically significant BCVA vision between the two groups(P >0.05). After 14 and 28 days of treatment, the treatment effective rate and cure rate of patients in the research group were significantly higher than those in the control group(χ^(2)=9.9175, 14.3261,17.1643,26.3243, P <0.05).After 28 days of treatment, the recurrence rate of 5.88% in the research group was significantly lower than 30.00% in the control group(χ^(2)=5.5138, P <0.05).Conclusion For patients with severe limbal corneal disease, the combined use of olopatadine, 0.1% fluoroMi Long eye drops and tobramycin dexamethasone eye ointment can significantly improve the clinical efficacy, with a high cure rate and low recurrence rate, which is worthy of promotion.
