集束化护理干预对重症脑卒中肠内营养相关性腹泻的影响

The Effect of Cluster Nursing Intervention on Enteral Nutrition-related Diarrhea in Severe Stroke

目的 探讨集束化护理干预对重症脑卒中肠内营养相关性腹泻的影响。方法 将2019年8月-2020年12月在广东省深圳市龙岗中心医院接受肠内营养支持的90例重症脑卒中患者按抽签法分两组,各45例。对照组进行常规护理,研究组于对照组基础上进行集束化护理,对比两组肠内营养相关性腹泻发生率、机械通气时间、ICU住院时间、并发症情况及患者满意度。结果 研究组肠内营养相关性腹泻发生率为8.89%,低于对照组的28.89%,差异有统计学意义(P <0.05)。研究组机械通气时间、ICU住院时间分别为(12.63±1.74)d、(14.14±1.55)d,对照组分别为,(14.93±2.10)d、(17.89±1.83)d,研究组机械通气时间、ICU住院时间均短于对照组,差异有统计学意义(P <0.05)。研究组的并发症发生率为6.67%,低于对照组的26.67%,差异有统计学意义(P <0.05)。研究组患者满意度为97.78%,高于对照组的77.78%,差异有统计学意义(P <0.05)。结论集束化护理干预的实施可有效减少重症脑卒中患者肠内营养相关性腹泻的发生,促进其身体康复,并有助于患者满意度的提升,值得临床推广。

Objective To explore the effect of clustered nursing intervention on enteral nutritionrelated diarrhea in severe stroke. Methods The 90 severe stroke patients who received enteral nutrition support in our hospital from August 2019 to December 2020 were divided into two groups according to the lottery method, with 45 cases in each group. The control group received routine care, and the study group performed clustered care based on the control group. The incidence of enteral nutrition-related diarrhea, mechanical ventilation time, ICU stay, complications and patient satisfaction were compared between the two groups. Results The incidence of enteral nutrition-related diarrhea in the study group was 8.89%, which was lower than 28.89% in the control group(P < 0.05). The duration of mechanical ventilation and ICU stay were(12.63±1.74) d,(14.14±1.55) d, and(14.93±2.10) d and(17.89±1.83)d, the duration of mechanical ventilation and ICU stay were shorter than the control group(P < 0.05). The complication rate in the study group was 6.67%, which was lower than the26.67% of the control group(P <0.05). The patient satisfaction rate of the study group was 97.78%, which was higher than 77.78%of the control group(P < 0.05). Conclusion The implementation of clustered nursing intervention can effectively reduce the occurrence of enteral nutrition-related diarrhea in patients with severe stroke,promote their physical recovery, and contribute to the improvement of patient satisfaction. It is worthy of clinical promotion.
