探讨二次妊娠合并瘢痕子宫产妇剖宫产并发症情况分析

Explore the Analysis of Complications of Cesarean Section in Women with Scarred Uterus During the Second Pregnancy

目的分析二次妊娠合并瘢痕子宫产妇进行剖宫产的并发症发生情况。方法选取乌兰察布市中心医院2019年7月-2021年7月100例行剖宫产的二次妊娠产妇作为主要研究对象,包括50例瘢痕子宫产妇(观察组)、50例非瘢痕子宫产妇(对照组),将两组产妇手术指标、新生儿情况和并发症情况等进行对比。结果观察组术中及术后出血量均高于对照组,恶露时间与分娩时间均长于对照组,差异具有统计学意义(P<0.05);观察组新生儿Apgar评分低于对照组,差异具有统计学意义(P<0.05);两组新生儿体质量差异无统计学意义(P>0.05);观察组并发症率高于对照组,差异具有统计学意义(P<0.05);产前出血量≥240mL、瘢痕子宫形成距二次妊娠<24个月为二次妊娠合并瘢痕子宫产妇发生并发症的主要影响因素,差异具有统计学意义(P<0.05)。结论二次妊娠产妇中,有瘢痕子宫问题的产妇不仅实施剖宫产手术需要更长时间,造成更多出血,而且术后的并发症风险更高,新生儿状况可能更差,母婴健康与安全需要更加重视,而二次妊娠时间距瘢痕形成时间较短、产前出血量较多等属于产妇并发症多发的危险因素,需要加以重视。

Objective To analyze the complications of cesarean section in women with scarred uterus during the second pregnancy.Methods From July 2019 to July 2021,100 second-pregnancy women who had undergone cesarean section were selected as the main research objects,including 50 women with scarred uterus(observation group)and 50 women with non-scarred uterus(control group),compare the two groups of maternal surgery indicators,neonatal conditions and complications.Results The intraoperative and postoperative blood loss of the observation group was higher than that of the control group,and the time of lochia and delivery were longer than those of the control group.The difference was statistically significant(P<0.05).The Apgar score of the newborns in the observation group was lower than that of the control group.The difference was statistically significant(P<0.05).The difference in body weight between the two groups of newborns was not statistically significant(P>0.05).The complication rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Prenatal blood loss≥240 mL,scarred uterus formation less than 24 months from the second pregnancy are the main influencing factors for the complications of the second pregnancy with scarred uterus,and the difference is statistically significant(P<0.05).Conclusion Among the second-pregnancy women,women with scarred uterus not only need longer time to perform cesarean section,which causes more bleeding,but also have a higher risk of postoperative complications,the condition of the newborn may be worse,and the health of the mother and baby and safety needs to be paid more attention to,and the time between the second pregnancy and the scar formation is shorter,and the amount of prenatal hemorrhage is higher,which are risk factors for multiple maternal complications,which need to be paid attention to.
