噻托溴铵联合糖皮质激素治疗慢性阻塞性肺疾病急性加重期的疗效及患者肺功能的影响分析

Analysis of the Efficacy of Tiotropium Combined with Glucocorticoids in COPD Exacerbations and Patients'Pulmonary Function

目的分析噻托溴铵联合糖皮质激素治疗慢性阻塞性肺疾病急性加重期的疗效及患者肺功能的影响。方法回顾分析广东省高州市人民医院2019年1月-2021年1月收治的60例慢性阻塞性肺疾病急性加重期患者,按治疗方式分组,其中30例接受噻托溴铵联合糖皮质激素治疗(观察组),另30例接受常规治疗,即常规抗感染、吸氧、糖皮质激素治疗,比较治疗效果,如疗效、肺功能、血气指标、炎症因子指标等。结果观察组总疗效96.67%高于对照组总疗效66.67%,组间数据有统计学意义(P<0.05),比较肺功能指标,治疗前组间数据无统计学意义(P>0.05),治疗后,观察组FEV_(1)占预计值百分比、FVC、FEV_(1)高于对照组,组间数据有统计学意义(P<0.05),比较血清学指标,治疗前组间数据无统计学意义(P>0.05),治疗后,观察组IL-6、CA-125、hs-CRP低于对照组,组间数据有统计学意义(P<0.05)。结论临床治疗慢性阻塞性肺疾病急性加重期,可考虑采用噻托溴铵与糖皮质激素联合治疗,患者肺功能、血清学指标等均可得到明显改善,疗效理想。

Objective To analyze the efficacy of tiotropium combined with glucocorticoids in the treatment of acute exacerbations of chronic obstructive pulmonary disease and the effect of pulmonary function in patients.Methods A total of 60 COPD patients treated between January 2019 and January 2021 in our hospital were retrospectively divided into groups according to the treatment modality,of whom 30 received tiotropium combined with glucocorticoid therapy(observation group)and the other 30 received conventional therapy,that is,conventional anti infection,oxygen absorption,glucocorticoid therapy,to compare the treatment efficacy,such as curative effect,lung functionBlood gas indicators,inflammatory factors indicators,etc.Results The 96.67%of total effect in the observation group was higher than the 66.67%of total effect in the control group,the data between groups were statistically significant(P<0.05),the pulmonary function indexes were compared,the data between groups before treatment were not statistically significant(P>0.05),after treatment,the FEV_(1) in the observation group as a percentage of predicted value,FVC,FEV_(1) were higher than the control group,the data between groups were statistically significant(P<0.05),the serological indexes were compared,There was no statistical significance in the data between groups before treatment(P>0.05),and after treatment,the IL-6,CA-125,and hs CRP in the observation group were lower than those in the control group,and the data between groups were statistically significant(P<0.05).Conclusions The combination of tiotropium and glucocorticoids may be considered for the clinical treatment of COPD exacerbations,and patients'pulmonary function,serological parameters,and other parameters can be significantly improved,with ideal outcomes.
