基于ERAS理念的护理干预应用于高龄高危前列腺增生合并膀胱结石患者的效果

Effect of a Nursing Intervention Based on Eras Philosophy Applied to Older High Risk Patients with Prostate Hyperplasia and Bladder Stones

目的探讨基于快速康复外科护理(ERAS)理念的护理干预应用于高龄高危前列腺增生(BPH)合并膀胱结石患者的效果。方法选择台山市人民医院2019年1月-2020年12月收治的高龄高危BPH合并膀胱结石患者60例作为研究对象,按入院顺序分为对照组和观察组,每组各30例。对照组在围术期采用常规护理,观察组在围术期采用快速康复外科护理(ERAS)。对比两组术后恢复情况、围手术期情绪、并发症发生情况。结果观察组患者的术后首次排气时间、首次下床时间、膀胱冲洗时间、导尿管留置时间、住院时间均短于对照组(P<0.05),观察组患者在术后1d及出院时的焦虑自评量表(SAS)得分均低于对照组(P<0.05),尿路感染、肺部感染、膀胱痉挛、尿管堵塞的并发症发生率均低于对照组(P<0.05)。结论基于ERAS理念的护理干预有助于减少高龄高危前列腺增生合并膀胱结石患者术后的并发症,并改善患者在围术期的焦虑情绪,促进患者快速康复。

Objective To investigate the application of nursing intervention based on the concept of rapid rehabilitation surgical care(ERAS)for high-risk prostatic hyperplasia(BPH)of advanced age.Effect of combined bladder stone patients.Methods Sixty elderly high-risk BPH patients with bladder stone admitted to Taishan people's Hospital from January 2019 to December 2020 were selected as the study subjects and divided into control and observation groups,30 patients each,according to the admission order.The control group adopted routine care in the perioperative period,and the observation group adopted rapid rehabilitation surgical care(ERAS)in the perioperative period.Postoperative recovery,perioperative mood,and the occurrence of complications were compared between the two groups.Results The patients in the observation group had shorter postoperative first flatus time,first ambulation time,bladder irrigation time,urinary catheter retention time,and hospital stay than those in the control group(P<0.05),and the patients in the observation group had lower anxiety self rating scale(SAS)scores than those in the control group on postoperative day 1D and at discharge(P<0.05)However,the complication rates of urinary tract infection,lung infection,bladder spasm,and urinary tube blockage were lower than those of the control group(P<0.05).Conclusions Nursing intervention based on eras concept can help to reduce postoperative complications in high-risk patients with prostate hyperplasia and bladder stones in advanced age,and improve patients'anxiety in the perioperative period and promote rapid patient recovery.
