冷刀剪开在输尿管小于2cm狭窄治疗中的效果观察

Observation on the Effect of Cold Knife Incision in the Treatment of Ureteral Stricture Less than 2cm

目的探讨冷刀剪开治疗输尿管小于2cm狭窄患者的效果。方法选取2019年10月-2021年10月凉山彝族自治州中西医结合医院收治的43例输尿管狭窄患者作为研究对象,采用随机数字表法将其分为两组,23例采用专门定制输尿管镜冷刀剪开治疗的患者列为观察组,20例采用钬激光内切联合球囊扩张术治疗的患者列为对照组,比较两组患者的治疗有效率,记录其手术耗时、术中出血量、住院时间等围手术期临床指标,统计两组患者治疗后的并发症发生情况及狭窄复发情况。结果观察组总有效率91.30%(21/23)高于对照组70.00%(14/20),其手术耗时、术中出血量、住院时间均低于对照组,该组患者治疗后的并发症发生率8.70%(2/23)低于对照组30.00%(6/20),以上数据经对比差异均具有统计学意义(P<0.05);观察组患者术后病情未复发,对照组复发率为5.00%(1/20)。结论在专门定制输尿管镜冷刀剪开治疗小于2cm的输尿管狭窄患者疗效显著,该术式可降低对患者机体造成损伤并促进其术后恢复,经该术式治疗后患者术后发生并发症的风险更小,狭窄不易复发,此术式的有效性及安全性均较高,具有推广价值。

Objective To investigate the effect of cold knife cutting in patients with ureteral less than 2cm stenosis.Methods 43 patients with ureteral stenosis were selected in Liangshan Yi Autonomous Prefecture Integrated Traditional Chinese and Western Medicine Hospital from October 2019 to October 2021,and they were randomly divided into two groups.23 patients treated with customized ureteroscopic cold knife were included in the observation group,and 20 patients treated with holmium laser endotomy combined with balloon dilatation were included in the control group,to compare the treatment efficiency of the two groups,record the surgical time-consuming,intraoperative bleeding,hospitalization,and count the complications and recurrence of stenosis in the two groups.Results The total efficiency of 91.30%(21/23)was higher than 70.00%(14/20)in the control group.Its time-consuming,intraoperative bleeding and hospitalization were lower than the control group.The incidence of post treated complications in this group was 8.70%(2/23)lower than 30.00%(6/20),the above data were statistically significant(P<0.05).Patients in the observation group had no postoperative recurrence,and the control group’s recurrence rate was 5.00%(1/20).Conclusion In the treatment of ureteral stricture less than 2cm by specially customized ureteroscopic cold knife cutting,the curative effect is remarkable.This operation can reduce the damage to the patient’s body and promote its postoperative recovery.After this operation,the risk of postoperative complications is smaller,and the stricture is not easy to relapse.This operation has high effectiveness and safety,and has popularization value.
