摘要
目的 探究早期功能锻炼应用于急性脑卒中偏瘫肩手综合征患者的效果。方法 收集2019年1月-2020年6月就诊于我院的急性脑卒中合并肩手综合征患者56例,随机数字表发分为对照组和观察组,每组各28例。对照组予常规药物、护理治疗,观察组在对照组基础上予早期功能锻炼,比较两组患者治疗前后的疼痛评分、手指肿胀程度、上肢功能,治疗后3个月生活质量,不良反应发生情况。结果 治疗后观察组疼痛评分(NRS评分)、手指肿胀程度(8字缠绕维度差)明显低于对照组(P<0.05),上肢功能(STEF评分)明显优于对照组(P<0.05);观察组治疗后3个月生活质量各项评分(WHOQOL-BRIEF评分)明显高于对照组(P<0.05);两组患者均未出现明显不良反应。结论 早期功能锻炼可缓解急性脑卒中偏瘫肩手综合征患者疼痛、手指肿胀程度,改善患肢功能,提高患者生活质量,值得临床借鉴应用。
Objective To investigate the effects of early functional exercise applied to patients with hemiplegic shoulder hand syndrome with acute stroke.Methods A total of 56 patients with acute stroke and shoulder hand syndrome who visited our hospital from January 2019 to June 2020 were enrolled in this study,and randomLy divided into control and observation groups,with 28 patients in each group.The control group was given conventional medication,nursing treatment,and the observation group was given early functional exercise based on the control group to compare the pain scores,finger swelling degrees,upper limb function before and after treatment,quality of life at 3 months after treatment,and occurrence of adverse effects.Results After treatment,the pain scores (NRS scores),finger swelling (difference in 8-word wind up dimension) of the observation group were significantly lower than those of the control group (P<0.05),and the function of the upper limbs (stef score) was significantly better than that of the control group (P<0.05);The scores of each item of quality of life (whoqol-brief score) at 3 months after treatment in the observation group were significantly higher than those in the control group (P<0.05);No patients in either group experienced significant adverse effects.Conclusions Early functional exercise alleviates pain and the degree of finger swelling,improves the function of the affected limb,and improves the quality of life of patients with hemiplegic shoulder hand syndrome with acute stroke,which deserves clinical borrowing.
