颅底沟通性脑膜瘤引发的一例关于颅底肿瘤的思考

Thinking About a Case of Skull Base Tumor Caused by Communicative Meningioma of the Skull Base

目的 通过对颅底沟通性脑膜瘤疾病而导致的颅底肿瘤疾病进行分析归纳,从而进一步增加对颅底肿瘤疾病自身典型性病变情况的了解程度以及清除对于非典型性病变对应的诊治途径,从而实现增加对颅底肿瘤疾病的临床诊断治疗质量。方法 选取松原市中心医院神经外科2021年7月所治疗的1例由颅底沟通性脑膜瘤而引发的颅底肿瘤疾病患者,通过对其进行临床治疗资料回顾性分析总结,了解颅底肿瘤疾病在CT与MRI此两种影像与临床的诊断表现。结果 通过对患者实施CT与MRI检查方式可知,患者颅底肿瘤组织形态比较复杂,且对附近的组织结构造成的硬性也具有多样性。虽然检查影像存在一定的差异性,但是对于垂体大腺瘤以及脑膜瘤等少数的肿瘤疾病存在显著的特征性影像情况。且患者疾病术后病理被进一步诊断为脑膜瘤WHOⅡ级。结论 由于颅底肿瘤疾病其病变形态组织较为复杂多样,且对附近组织结构所产生的影响也存在多样性特点,究其原因,主要是受附近组织结构与肿瘤本身成长特点两者共同的影响。除此之外,综合性影像学检查方式还可以为临床诊断颅底肿瘤疾病给予正确合理的影像诊断思路方向,有利于临床诊治以及预后评价,值得临床推荐实施。

Objective To analyze and summarize the skull base tumors caused by skull base communicating meningioma diseases,and to explore the clinical features of skull base communicating meningioma,in order to further increase the understanding of the disease of skull base tumor and its own typical pathological changes,and clear the corresponding diagnosis and treatment of atypical pathological changes,therefore,the quality of clinical diagnosis and treatment of skull base tumor can be improved.Methods A case of skull base tumor disease caused by skull base communicating meningioma treated in neurosurgery of our hospital in July 2021 was selected.The clinical data were analyzed retrospectively,to investigate the clinical manifestations of skull base tumor in CT and MRI.Results The results of CT and MRI showed that the morphology of skull base tumor was complex,and the rigidity caused by adjacent tissue was also varied.Although there are some differences in imaging,there are significant imaging features in a few tumors,such as pituitary macroadenomas and meningiomas.The postoperative pathology of the patient was further diagnosed as meningioma grade WHOⅡ.Conclusion Because the pathological changes of skull base tumor are complex and varied,and the influence on the nearby tissue structure is also varied,it is mainly affected by the adjacent tissue structure and the growth characteristics of tumor itself.In addition,the method of comprehensive imaging examination can give correct and reasonable direction to the clinical diagnosis of skull base tumor,which is beneficial to clinical diagnosis and treatment and prognosis evaluation.It is worthy of clinical recommendation.
